At the July l3,1994 regular meeting,the Inland WetlandsAgency/Conservation Commission approved with conditions application#94—25W -IWA/CC permit for the construction of a single family houselocatedat1278MainStreet,A—40 zone as shown on plans prepared byJ.R.Russo &Associates,No.93117,dated June 6,1994 and revisedthroughJune21,1994.This approval is subject to the followingconditions.
l.Two blueprint copies of the entire set of plans amd thisletterreproducedthereon,must be submitted to thisCommission.This must be completed within 65 days of approval
prior to any construction activity on the site.
2.Construction activities shall be subject to the approval oftheTownEngineer.
3.The application shall indemnify and hold harmless the Town ofSouthWindsoragainstanyliabilitywhichmightresultfromtheproposedoperationoruse.
4.We recommend a bond in the amount of $4000.to be submittedtoPlanningandZoningCommissiontoensurecompliance withtheErosionandSedimentationControlMeasures.
5.The permit is valid for five years and shall expire on Julyl3,1999.
6.All approvals required must be obtained and submitted priortoanyactivityonthesite.
A contact person shall be identified directly on the plans.
8.The septic system design is subject to final review by theTownSanitarian.

9.The plans show standard rip rap on both sides of the proposedbituminouspavementintheareaoftheproposed72"R.C.P.The rip rap shall be extended beyond the outlet of the pipetoprovideadequateprotection.This shall be approximately10’X lO’and shall be shown on the final plans.
