The South Windsor Water Pollution Control Authority,at its Regular Meeting on June 4,2024,gave
approval for change in Nature of Wastewater from residential use to commercial use to the Town's
sewerage system,for the proposed 4 Elements Vitality Institute,a medical practice and wellness
sanctuary,more specifically shown on plans entitled,”Medical Office Building 1300 Sullivan Avenue
(Formerly 8 Collins Lane)South Windsor,Connecticut”,prepared by Design Professionals,South
Windsor,CT;Project No:5195,Sheet C-SPZ,Dated 3/27/24 and revised 5/24/24.
This approval letter must be superimposed on the final approval plans.
