Mr.Craig Entwistle
Paul C. Higgins Insurance Adjuster

RE: Claim No.: PR132341
Insured: Jim Melough
Location: 25 Valley View Drive, South Windsor,CT
Claim: Foundation Damage
AEGIS File No.: A13994
Dear Mr.Entwistle,
On May 17, 2016, AEGIS Engineering Services, Inc. (AEGIS)was retained to review the above
referenced claim. The objective of our assignment was to evaluate the cause of claimed damage
to the cast-in-place concrete foundation at the rear addition of the insured's house.
On May 26, 2016,AEGIS met at the property to observe the claimed damages and interview Mr.
Melough. On the same date, we reviewed the property file at the South Windsor Building
Department. The following is a summary of our findings.
Property Description and Loss Background
Mr. Melough has owned the subject property since 1991. The property is improved with a raised
g J P P Y
ranch that was built in 1973. A 24-foot by 26-foot addition was constructed at the left end of the
rear wall of the house in 1997. The rear addition is wood framed and is constructed on a cast-in-
place concrete foundation that encloses a fully finished basement that walks-out to the rear of the
house.
Mr. Melough stated that a few years ago he noticed some small cracks in the exterior face of the
concrete foundation at the rear left and right corners of the rear addition. He stated that he
recently became aware of news reports of deteriorating foundations in the area that were

•constructed with concrete supplied by the J.J. Mottes Company of Stafford, CT. Mr. Melough
stated that he then took a closer look at the exterior surfaces of the concrete foundation walls of
the rear addition and noticed more cracks.
Mr. Melough stated that he contacted the builder of the rear addition, and that the builder
informed him that the concrete used to build the foundation of the rear addition was supplied by
J.J.Mottes Company.
Review Findings
Due to drywall finishes in the basement, AEGIS was not able to evaluate the interior surfaces of
the foundation walls of the rear addition. Based on these finishes, it appears that the top of the
concrete foundation walls extend approximately 44-inches above the basement floor. Our
evaluation of the concrete foundation was limited to the exterior surfaces of the foundation walls
that extended above-grade.
The following was observed at the rear addition:
0—The rea undation-wall-extended-approximately-4 I-inches-above-grade-along-its-full ----
length. At the left side of the addition, the top of the foundation wall extended 38 inches
above grade at the front-left corner and 41 inches above grade at the rear-left corner. At
the right side of the addition, the foundation wall extended 4 inches above grade at the
front-right corner and 16 inches above grade at the rear-right comer.
• The exterior surfaces of all three of the concrete foundation walls at or near grade level
were covered with a pattern of interconnected horizontal and vertical cracks, commonly
referred to a"map-cracking" The cracks varied in width from hairline to approximately
3/16 inch.
• Visible map cracking was most severe at the rear left and right comers of the foundation,
and along the right foundation wall.
• The visible areas of the foundation walls did not appear to be leaning or bowed. There
were no interior finish damages in the basement that would indicate foundation
movement has occurred.
Common causes of map cracking in concrete include, but are not limited to alkali-silica-
reactivity (ASR); internal sulfate attack(ISA); and external sulfate attack(ESA). The primary
difference between internal and external sulfate attack is the source of the sulfate ions which
"attack" components of the concrete mixture and cause distress in the concrete. In ISA, the
sulfates were incorporated in the concrete mixture prior to placement. In ESA, the source of
sulfates is external (e.g., contained in the soil or groundwater), but the sulfate ions gradually
leach into the concrete via groundwater or surface water transport. In short, in ISA the concrete
itself is defective while in ESA harmful chemicals from the environment leach into otherwise
sound concrete via groundwater or surface water transport.
It is our understanding that distress, in the form of map cracking, of many cast-in-place concrete
foundations in the region has been linked to J.J. Mottes, who obtained crushed stone aggregate

containing a deleterious mineral known as Pyrrhotite from Becker's Quarry in Willington, CT.
Pyrrhotite is an iron sulfide. When exposed to moisture and oxygen, the Pyrrhotite oxidizes and
expands in volume, cracking the crushed stone aggregate and the surrounding cement paste,
which leads to visible map cracking and eventual failure of the concrete.
Further testing would be required to determine the exact cause(s) of the distress. Testing would
include core sampling and petrographic analysis of the concrete that comprises the foundation,
and possibly testing of soil and groundwater.
Conclusion
The observed map cracking and deterioration of the concrete are characteristic of underlying
problems with the concrete mixture, or external sulfate attack via groundwater transport. There
are several potential defects in the concrete that could result in the observed damages.
The deterioration of the concrete could be due to deleterious materials that were introduced into
the mixture at the time of production (i.e.ASR, ISA, or aggregate containing Pyrrhotite), or due
to sulfates from the surrounding environment that were introduced into the concrete via
CP ndwater-or surface-water-transport-after_-it_was-placed-(i.e.,r-ESA). Tbe-Aack-of-nearb-y------
wetlands or a septic system (common sources of sulfates within the environment) immediately
adjacent to the deteriorated areas of the insured's foundation suggests that external sulfate attack
is not a likely cause.
The insured believes that the concrete used to construct the foundation of his house in 1997 was
supplied by J.J. Mottes. The map cracking observed in the foundation walls is consistent with
oxidation of Pyrrhotite, a deleterious mineral that has been linked to aggregate in concrete
supplied by J.J. Mottes to the Stafford Springs area in the 1980s and 1990s (when the rear
addition house was built).
Collection and laboratory analysis / testing of core samples (including petrographic analysis),
and possibly sampling and analysis of soil and groundwater would be necessary to determine the
exact cause(s)of the observed damage to the concrete.
It should be noted that our findings, conclusions, and recommendations are based on our
observations during our site visit. Unless expressly stated otherwise, interior finishes were not
removed and contents were not moved to observe and evaluate all areas of the structure. Our
conclusions concerning extent of damages and necessary repairs are, therefore, limited to the
degree that structural elements of the building were readily accessible and visible at the time of
our inspection.
Our opinion(s) concerning the necessary scope of repairs have been prepared for estimating
purposes only and could change if further intrusive investigation by AEGIS or others reveals that
the damage is more extensive than indicated by our initial limited inspection or if the local
building official requires code upgrades that are not anticipated in the repairs. AEGIS is not
serving as the engineer of record for repairs. If required by the local building official or by
regulation, the insured should retain their own engineer to develop an appropriate repair plan.
