and conditions of the change shall prevail.
ACCEPTANCE OF SALES AGREEMENT:
All material is guaranteed to be as specified.All work to be completed in a professional manner according to standard practices.Any alteration or deviation from the above
specifications involving extra costs will be executed only upon written orders and will become an extra charge over and above the agreement.All agreements are contingent
upon strikes,accidents or delays beyond our control.Owner to carry fire,tornado,and other necessary insurance.Our workers are fully covered by workman's compensation
insurance.System performance cannot be guaranteed if any components of the existing distribution system are hidden,inaccessible or otherwise do not meet the
requirements of the new cooling or heating equipment being Installed.New equipment performance cannot be guaranteed when matched with existing equipment.
The prices,specifications and conditions are satisfactory and hereby accepted.Aiello Home Services is authorized to do the work as specified.Payment in full is due upon
completion(C.O.D.)unless otherwise specified above.A finance charge of 1.750.b per month(21%per annum)will be added to all past due accounts.The undersigned agree
to pay reasonable attorney's fees,court costs and collection fees in the event of legal action and authorize Aiello Home Services to perform a credit check if necessary.
