The fire was reported by the occupant Olgiard Lisowski of 103 Strawberry Lane. Location: The fire occurred at 103 Strawberry Lane, in South Windsor, CT. The structure is a 3 unit condo building that is approximately 6,745 square feet. Unit 103 is the end unit on the North. The unit is 1,869 square foot condo with a 504 square foot attached garage. Date &Time of Investigation: This investigation commenced at 14:30 hours on 03/02/06 by Fire Marshal Walter Summers of the South Windsor Fire Marshal's Office and continued until 22:00 hours on 03/02/06. Scene was reexamined on 03/06/06 by Fire Marshal Summers, Michael J. Driscoll of PT&C Forensic Consulting Services, P.A., & Sergeant Taft of the South Windsor Police Services. Summary& Scope: On 03/02/06 at 14:30 hours, Fire Marshal Summers was assigned to complete an origin and cause investigation by South Windsor Fire Department Captain Kevin Cooney, at the above stated location. Assisting the investigation were members of the South Windsor Police Services and the Connecticut State Fire Marshal's Office. South Windsor Fire Department Fire Captain Kevin Cooney stated that on 03/02/06 at 14:03 hours, the fire department dispatcher sounded the alarm for a reported structure fire at 103 Strawberry Lane. First responding units from the South Windsor Fire Department reported a working fire in the garage of the structure at 14:06 hours. Fire Marshal: South Windsor Fire Marshal Walter Summers responded to the scene on 03/02/06 at 14:30 hours and conducted an initial scene assessment. Assignment: At approximately 14:30 hours on 03/02/06, Fire Marshal Summers arrived at the scene with an investigative car. Chief Building Official Chris Dougan responded to the scene. Weather. At the time of this fire, it was reported, and Fire Marshal Summers observed the following weather conditions to be snowing, light winds at 6.9 mph from the NNE and a temperature of 26.6 Degrees F. The weather did not play a significant role in the growth and development of this particular fire. Description: The involved structure is an 1869 square foot 2 and one half story condo unit with a 504 square foot attached garage. The exterior consisted mainly of vinyl siding. It also contained an attic space. The foundation was constructed of poured concrete with full walk-out basement. The roof was gabled with asphalt type shingles and wood underlayment. Electrical Service: The underground electrical service was located on the structure's North side and the main panel was located on the wall inside of the basement the North side. Fuel Load: The main fuel load in this fire consisted of ordinary combustibles including but not limited to, plastic garbage can and recyclable bin, clothing, paint and aerosol cans, mattress, shoes which made up the contents within the structure. The area shows signs of the lowest burning and most extensive damage. The determination was based upon the physical evidence available at the scene at the time of this investigation, including bum patterns, lines of demarcation, fuel loads, areas of most extensive damage, and occupant statements. Flame Spread: Based on bum patterns, lines of demarcation and physical evidence at the scene, the fire traveled from the area of origin and went upward and outwards along the path of least resistance. Sprinklers: A sprinkler system was not in service at the time of this fire. Smoke Detectors: Smoke detectors were not present inside the garage. Smoke detectors were present in the occupiable space and operated. Injuries: There were not injuries in this fire. Scene Security: Prior to the arrival of Fire Marshal Summers the scene was secured by members of the South Windsor Fire Department. Suppression activities were on going and no origin and cause investigation was started until they were completed. Scene security was maintained throughout the duration of Fire Marshal Summers' scene examination/investigation by the South Windsor Police Services. Fire Marshal Summers called the South Windsor Police Services for the Command Vehicle and an Agent to respond. Also requested was a representative of the State Fire Marshal's Office. After meeting with the incident commander exterior examination was commenced. Exterior Examination: An exterior examination was completed which revealed the following: The East side of the building revealed heavy damage to the overhead door to the garage. The door was laying on the garage floor. The structural members showed sign of heat damage but were intact. See photo 1 A partially melted garbage container and partially melted recyclable bin were located outside the garage. Both items showed signs of heat damage. The North side of the building revealed fire damage around the 2 window openings located in the garage walls. The rest of the structure showed signs of smoke damage to the eave vents. Vinyl siding is damaged at the top of the wall and the areas around the windows of the garage. The electrical service is located on the exterior wall here. The Fire Department used the disconnect located next to the meters to turn the power off to all the units. Vinyl siding is in tact. See photo 4 The West side of the building showed heat and smoke damage to the eave vents and the vinyl siding at the top of the wall. See photo 2 A tan colored Mercedes parked in the driveway showed heat damage to it by the lower rocker panel by the driver's door. See photo 7 Windows & Doors: The overhead door to the garage was on the floor of the garage. The Owner stated that he had opened it to remove a car from the garage during the fire. The front door located on the East side was intact. The rear door located on the West side was intact. The door between the garage and the living space was closed at the time of the fire and the metal covering was on the front lawn. Windows on the North side of the garage were destroyed by the fire. Only the window casing was left. One North wall window in the Kitchen was broken by the fire department during overhaul. All other windows were intact. Interior Examination: Following the exterior examination, a systematic examination of the interior section of the structure was commenced. Starting with the least damage area and working towards the most damage area, this investigator started in the basement of the structure. The area revealed no smoke or heat damage. The area was also under renovations with rough sheetrock walls and rough electrical work installed. The area revealed extensive storage of normal household contents including but not limited to clothing and furniture. The furnace room was located and the gas fired forced hot air furnace revealed no damage. The room also contained extensive storage. The electrical room was found and this room also contained extensive storage. The electrical panel was opened. All circuits were on except the following three circuit breakers had tripped: Circuit#1 15 amp GFCI Circuit#6 20 amp Garage Circuit#15 15 amp Garage Refrigerator Moving up the basement stairs to the living space, I noticed that all the sheetrock had smoke staining on it (see photo 14). The staining got darker as you moved towards the front vestibule. The occupant's belongings and furniture were intact and appeared to be located in normal positions. The front vestibule contained the front door and the door to the garage. The sheetrock in the area had holes in it from the fire department overhaul operations. The structural members did not reveal any damage. The garage door frame was examined and it revealed more damage to the garage side of the frame then the living space side. The inside of the garage was examined. All sheetrock was off the framing. The wall framing was intact and showed little sign of heat damage. The roof rafters showed signs of heat damage with the most extensive damage to the draft stop plywood located at the wall between the garage and the living space. The windows on the North side were missing. The west floor area had a refrigerator and other storage. At this time, it was determined that the garage was the room of origin. Interviews were then conducted with the three occupants of the house. Interviews were conducted separately by members of the South Windsor Police (see police report for statements). They state that the mother and son went to Rockville Court this morning. When she arrived home around 11:00 hours she pulled her car, a 2 door tan colored Mercedes, into the garage, nose first and closed the overhead door. The husband arrived home a little later and parked in the driveway behind the Mercedes. At approximately 14:03 hours, the husband goes to the garage to get a drink from the refrigerator, and notices smoke in the garage and a small fire by the front of the garage southeast side. He notifies the occupants of the house and opens the garage overhead door. He proceeds down the southeast wall and out of the garage to move his car. He comes back to the garage and gets into the Mercedes from the passenger side and moves the Mercedes out. He states that he cannot get into the drivers side of the Mercedes due to storage of clothing, shoes, paint cans, etc.. At this time the son states that he comes to the garage door and witnesses his father moving the garbage can and recyclable bin out of the garage. The son tries to fill up a container with water to put the fire out, but when he comes back the fire is too big. The Fire department arrives at 14:06 hours and reports a fully involved garage fire. The Mother states that the Father and Son do smoke in the garage. The Father states that the ash can is in the front Northeast comer. The occupants also state that the electrical outlet on the South wall has been having problems. Around 16:50 hours, Detective John Kananowicz from the State Fire Marshal's Office arrives. I briefed him on the fire, reviewed the statements of the occupants, and toured the scene. Based on this meeting the electrical system in the garage was observed first. The overhead lighting is provided by two incandescent lights. The outlet on the west wall was observed next. The plastic cover was gone but the duplex outlet was intact and showed signs of exterior heat damage. The outlet box was intact and showed signs of exterior damage to the unprotected side. The side protected by the sheetrock was in good shape. The outlet on the South side was observed next. This outlet was gone except for the metal piece of the outlet. The grounding wire was still attached to the screw. The wires were observed next. The wires were in good shape with no pitting or beading. The outlet box showed signs of fire damage to the unprotected side and some compromise to the protected side. The interior of the box showed no signs of internal heat damage. The stud that it was attached to showed no signs of interior heat or flame damage. The top plate of the cavity was intact and the firestopping caulk around the wiring was intact. The electrical outlet was ruled out as the cause of the fire. Based on the son's statement the garbage can and recyclable bin were observed next. They revealed melting on one side of each. The father stated that the two were stored next to each other. Inside of the recyclable bin were found cigarette boxes and used matches. The melting occurred at the bottom of each and progressed upward and outward up the garbage can and recyclable bin. Based on the occupant's statements along this South wall were racks full of clothing, crafts, paint cans, aerosol cans, etc. in close proximity to the garbage can and recyclable bin. At this time, the area of most extensive fire damage was identified as the area were the garbage can and recyclable bin was located on the South wall of the garage. A systematic, layered examination was then undertaken so as to determine the cause of this fire. This area revealed to contain remenants of clothing, paint cans, aerosol cans, shoes, craft items, and melted plastics on the floor located in the southeast section of the floor. This area also lines up with the damage found on the Mercedes. This investigator and other investigators have identified this as the area of origin. Photos were taken by the State Fire Marshals Office. The garbage can and recyclable bin were inspected. The recycle bin contained some cigarette boxes and used matches. The interior examination was concluded at 21:00 hours on 3/02/06. The homeowner was interviewed again and stated that the ash can was located on the other side of the garage and that he was not smoking in that area. After reviewing the findings of this investigation with all investigators assigned to the same, the building was turned over to the owner. SOUTH WINDSOR CT., 06074 860-644-2511 EXT 217 CASE NUMBER IN TYPE INSURANCE 2006-00099 Structure fire REPORT SENT SUPPLEMENTARY RE-OPEN nASSIST CLOSING This investigator and the other investigators assigned to the investigative team also examined all other possible ignition and heat sources which may have resulted in this structure fire and have concluded that this fire was undetermined in nature at this time. On 3/6/06 at 11:45 hours this investigator along with Sergeant Taft arrived on scene at the request of Michael Driscoll, CFEI of PT&C Forensic Consulting Services, and property manager Ms. McCarron to go over our findings. Mr. Driscoll stated that he had Mrs. Lisowski place the garbage cans and recyclable bin in place. She also stated that the ash can was located in that comer and not the opposite comer as stated by the husband. Mr. Driscoll stated that he found the ash can in that area. Property Manager McCarron stated that she spoke to electrician who came out to research the outlet problem. The electrician stated that the GFCI breaker had tripped. He checked the outlet and reset the breaker. No problem found in the outlet. Mr. Driscoll stated that his determination of the cause of the fire will be careless disposal of smoking material. Conclusion: The fire that occurred within the structure identified as 103 Strawberry Lane, South Windsor, Ct. on 3/06/06 at 14:03 hours was undetermined as determined as follows: The examination of the fire scene and the elimination of all incendiary and natural sources of ignition. Based on the above described bum patterns, lines of demarcation and the physical evidence at the scene, as well as witness statements it has been determined that this structure fire was undetermined. The exact source of ignition was undetermined. Case Closed: No Criminal aspect/Suspended/Arrest. Will file SFMO report when received.
