South Windsor, CT Contractor: PDS Engineering Project No.: 00512C Weather:partly sunny Date: July 21, 2000 Purpose:To provide construction monitoring services to evaluate by observation and/or by testing whether work generally conforms to project plans. Contractor Activities: Upon this writer's arrival, it was observed that Paul Morin Construction prepared the area shown on the attached Field Sketch. The foundation footing and rebar appeared to conform to project plans and specifications. Subsequently, 8-cubic yards of 3,000-psi concrete was placed by means of the truck chute, then manually worked into place and finished with hand trowel. JGI Activities: Reinforcing steel was reviewed prior to today's concrete placement. The steel was observed to be placed in general accordance with project drawings. One set of Concrete compressive strength test cylinders was fabricated from today's concrete placement in accordance with ASTM C 31. Refer to the Concrete Strength Test Summary of this date for details. The General Contractor was notified of these results prior to JGI's departure from the site. Prepared by: B. Contractor Activities: Upon this writer's arrival, it was observed that Paul Morin Construction prepared a foundation wall for concrete placement. The rebar appeared to conform to project plans and specifications. Subsequently 9-cubic yards of 3,000-psi concrete was placed by means of the truck chute, vibrated and finished with a hand trowel. JGI Activities: Reinforcing steel was reviewed prior to today's concrete placement. The steel was observed to be placed in general accordance with project drawings. One set of Concrete compressive strength test cylinders were fabricated from today's concrete placement in accordance with ASTM C 31. South Windsor,CT Contractor: PDS Engineering Project No.: 00512C Weather:sunny Date: August 3, 2000 Purpose:To perform field density testing. Contractor Activities: Upon this writer's arrival, it was noted that there was no contractor or representative present on site. Observations indicated that the slab on grade had been prepared for slab placement. Compactive efforts were not witnessed by JGI. This writer performed density tests on the interior slab on grade sub-soils. JGI Activities: Six field density tests were performed on previously prepared earthwork using a nuclear density gauge in general accordance with ASTM D 2922. The test results indicated that the specified 95% minimum compaction requirement had been achieved as compared to ASTM D 1557 test methods. Please refer to the attached Field Density Test Summary for individual test data. Locations of today's project activities are illustrated on the attached Field Sketch. Prepared by: M.
