Contractor Activities: Upon this writer's arrival, Ernie's Crane Service had begun excavation of approximately 30- inches of native material for the placement of Red-brown, C-F Gravel, C-F Sand, trace Silt to be used as the parking lot subbase. The native subbase was proofrolled by a vibratory roller prior to backfilling. Backfilling began with an 8-inch lift of processed gravel followed by a 6-inch lift of stone. A compactive effort was applied at the completion of each lift with a vibratory roller. Density testing was performed with the use of a Humbolt nuclear gauge. JGI Activities: Five field density tests were performed on today's earthwork using a nuclear density gauge in general accordance with ASTM D 2922. The test results indicated that the specified 95% minimum compaction requirement had been achieved as compared to ASTM D 1557 test methods. Please refer to the attached Field Density Test Summary for individual test data. Locations of today's project activities are illustrated on the attached Field Sketch. Prepared by: S. Project: SBA Tower 10125-087 Client: Dicin Electric South Windsor, CT Contractor: DRP Construction Project No.: 01592C Weather:Cloudy, 51 degrees Date: December 13, 2001 Purpose:To review concrete construction, fabricate specimens for strength testing, and observe placement of reinforcing steel. Contractor Activities: D.R.P. Construction previously installed concrete forms and reinforcing steel for the pier column of the proposed cell tower. The steel and forms appeared to have been properly secured and geometrically correct. Subsequently, 8-cubic yards of 5,000-psi concrete was supplied to the site by Builders Concrete. The concrete was placed at the prepared locations by the delivery truck chute, it was mechanically vibrated into place and finished by hand trowel. JGI Activities: One concrete compressive strength test set was fabricated from today's concrete placement in accordance with ASTM C 31. Refer to the Concrete Strength Test Summary of this date for details. Reinforcing steel was reviewed prior to today's concrete placement. The steel was observed to be placed in general accordance with project drawings. Locations of today's project activities are illustrated on the attached Field Sketch. Prepared by: C. JGI Activities: This writer performed a visual inspection of the above named tower erection. The intent of this inspection was to verify that the worked performed was completed in general accordance with project specifications, engineer approved drawings, and applicable building codes. The remaining carrier platforms had been installed as per project specifications. Bolting had been torqued and the safety line had been installed. No deviations were open at this time. No further inspections are scheduled. Prepared by: D. O'Keefe AWS/CWI 94110511 Reviewed by: ;!%F Date: 2/8/02 i 4• orCoJV A 4• A • _•8 lyY eta y9 • ONA;C-00.
