Davies, DTE Re; 12 Seapave Road, as-built review, 8-26-91 In response to your request to review the improvements at this address for a Certificate of Occupancy recommendation, I inspected the site and at this time cannot approve this site. The first request for an inspection came to this office about 1-09-91 and was rejected for a number of reasons. They included such things as; 1. The broken bituminous curb, still hasn't been replaced. 2. The utility trench repair, has not been done. 3 . The concrete bumper blocks, have not been .installed completely or properly secured. 4 . The 10" ADC pipe was not installed as proposed and will not function properly. 5 . The existing pipe under Seapoave Road has not been cleaned. 6 . The sanitary sewer information is still incomplete.
