8. An As Built plan is required for final site inspections prior to issuance of a certificate of occupancy.
The as-built plan must include all applicable information listed on the attached as-built checklist.
9. All pipe and structures to be backfiilled with free draining material compacted in maximum one foot
lifts with approved compaction equipment or approved equal if deep trench is encountered. Any pipe
or structure work requires inspection prior to backfilling.
10.Any problems and/or questions regarding construction of this project will be directed to McShane
Construction in the field at phone 860-847-292-4300.
11.All filling to be done in accordance with Department of Transportation specifications. To be
compacted in maximum lifts and tested as necessary.
12.Job meetings will be held as needed, and can be called by the developer or Town.
13. Seventy-two (72) hours' notice needed for certificate of occupancy inspection and a $35 C.O.
inspection fee is due at that time.
14.Any proposed changes are to be presented to the Engineering or Planning Department prior to
implementation on change order form in quadruplicate with 4 copies of the drawing. Town will
attempt to turn around within 24 hours.
15.Normal hours of operation 7:00 a.m.—5:00 p.m.,no work on Sunday.
16.DOT permit needed for all work within state road. Copy must be given to the Engineering
Department.
17.A street excavation permit is required from the Town of South Windsor for the driveway and
widening of Kennedy Road and utility cuts in Kennedy Road.
18.A drain layers permit is needed from the Town of South Windsor for the sanitary sewer lateral and
connection on this site;the sewer connection charge is due before this permit is issued.
19.No paving after October 15 or before April 15 without written approval from Director of Public
Works.
20.Extra care must be taken when compacting around structures such as catch basins and manholes.
These spots often become problem areas in the future due to settling, therefore, the town
staff/inspectors will be requesting additional compaction tests in these areas.
21.Inspection of gravel access road required prior to placement of topsoil.
