ADDITIONAL REMARKS
right. Operator 2 started to pass Vehicle 1 on the left when Vehicle 1 turned left into her path. Operator 2 struck the
left front of Vehicle I with the right front of her vehicle.
CONCLUSION: Based on my investigation at the scene,the dams ge.to the vehicles and the verbal statements of the operators,
I concluded the following events occurred resulting in the crash: Operator I was traveling eastbound on Chapel Rd.and
-N,reparing to turn left into Merwin Mason Supply. Operator 1 turned on his left turn signal which was inoperative to the
rear. Operator I swung slightly to the right prior to turning left. Operator 2 was following Vehicle 1. Operator 2 saw
Vehicle 1 slowing down and due to Vehicle 1's inoperative turn signal did not realize that Vehicle 1 was intending to turn
left. Operator 2 crossed over the double yellow no passing lines and attempted to pass Vehicle 1 on the left. Vehicle 1
turned left and Operator 2 struck the front left of Vehicle 1 with the front right of Vehicle 2. The owner of Vehicle 1,
Don Childree General Contractors,was issued an infraction for violation of C.G.S. 14-96e,Fail to have Turn Signal. Operator
2 was issued an infraction for violation of C.G.S. 14-234,Passing in a No Passing Zone. Vehicle 1 was removed by Caron
Auto. Operator 1 left with a co-worker. Operator 2 and Vehicle 2 were removed by South Windsor Garage.
At approximately 1130 hirs,the owner of Don Childree General Contractors called to say that he learned that Allen's
insurance policy with Premier had been cancelled. I spoke with Operator 2 and she confirmed that her insurance coverage
had been cancelled. Operator 2 stated she learned this today after the crash. I confirmed that her policy was cancelled
with Premier(see attached.) I met with Operator 2 at her workplace,Admiral Moving&Storage,420 Ellington Rd. Operator
2 was issued a Misdemeanor Summons for violation of C.G.S. 14-213b,Operating a Motor Vehicle without Minimum Insurance,
and given a court date of 140CT08 at Manchester Superior Court. Operator 2 was released on scene with a$500.00 non-surety
bond approved by Agt.Duchesne#147.
Case closed by arrest.
