The released oil spread across the floor of the basement from the southeast corner to the northwest corner into a floor pit indicated on the Site Sketch(not to scale)included in Appendix A with this report. After an initial surface abatement by others,Enviroshield,Inc.was subsequently retained to investigate and abate the sub-slab impact to soil and groundwater. All work was completed in consideration of the integrity of the structure in accordance with a structural engineer who had visited the site and supplied recommendations for safe digging. A copy of the GNCB Engineers safe digging recommendation is included in Appendix D with this report. All work performed as part of this investigation was conducted or overseen by Enviroshield,Inc.,a State of Connecticut Department of Energy and Environmental Protection(CT-DEEP)licensed spill cleanup contractor using industry standard equipment and methods and in consideration of CT-DEEP standards,recommendations and residential guidelines. The subject property is a single-family residence and is serviced by municipal water. The following is a chronological discussion of the work performed by Enviroshield,Inc.at the subject site since initiation of the site work on Thursday,August 4,2011. Discussion: Thursday,August 4, 2011 Enviroshield installed 1"probe holes in the basement and in two exterior areas outside of the basement to discover the extent of fuel oil impact to the soil and/or groundwater under the basement floor and outside of the foundation foot print. The probe points were identified as Hand Probe(HP)1—HP8 and GeoProbe®(GP)9 and GP 10 as indicated on the Site Sketch in Appendix A. Field screening and visual observation during probing indicated oil impact in the area of HP4 and HP8. A single soil sample was collected from HP7 for disposal profile and confirmation of fuel oil impact between 2' and 6' below the basement floor. The CT-DEEP standard for Extractable Total Petroleum Hydrocarbons(ETPH)is 500 parts per million (ppm). As indicated in the attached CET Analytical Report(CET#11080546)included in Appendix C with this report,the soil under the floor at the area of HP7 contained 6200 ppm of ETPH. Oil impact was confirmed existing under the slab of the concrete floor. P.O. Tuesday,September 6, 2011 Using an air driven&electric jackhammer,Enviroshield staff began the braking up and hand excavation of cement in the area of HP 4 and HP7&8,near the area of the floor pit. The area of excavation is indicated on Site Sketch;a blower system was installed to evacuate fuel oil odors from the home during the work. Wednesday, September 7, 2011 Enviroshield staff along with a support contractor removed and excavated and disposed oil impacted soil and vacuumed and legally disposed oil impact water and fuel oil from under the basement floor. During this event,7.41 tons of solid material was removed and disposed at Aggregate Industries,a Massachusetts DEP licensed solid disposal facility. A copy of the weight slips and Material Shipping Record&Log is included in Appendix B with this report. A total of 65 gallons of oil impacted water was also removed and legally disposed at Bridgeport United Recycling(United),a liquid disposal facility located in Bridgeport,Connecticut. A copy of this and all referenced Bill of Ladings noted in this report are contained in Appendix B. During the removal of oil impacted soil,a 6"diameter well(RW 1)was installed to continue abatement of oil impacted water. The well was set 7'deep where indicated on the Site Sketch. The fmal dimension of the excavated area around HP4 was 3'x 2'x 4' deep to groundwater;the dimension of the HP7—HP8 area was 10'x 4'x 4' deep to groundwater. Thursday,September 8,2011 Enviroshield staff backfilled around RW 1;A second 6"well was installed at the area of HP4(RW2)and was installed 5' below surface grade. The bottom of the HP4 excavated area was sampled for analysis and the excavation in that area(around RW2)was backfilled to grade. Two additional micro wells were installed at the former points identified as HP5 (HPSA)and HP6(HP6A). A soil sample from HP6A was collected from just above the groundwater table between 3'to 4'below grade. As indicated in the CET Analytical Report(CET#11090192)included with this report in Appendix C,no ETPH was reported present above the Connecticut Department of Energy&Environmental Protection(CT-DEEP)standard of 500 parts per million. A total of 400 gallons of oil impacted groundwater was vacuumed from RW 1 and was disposed at United. September 9-12, 2011 Enviroshield staff collected samples from the opened excavation between HP7 and the Floor Pit.The soil samples were collected near the groundwater interface from the north,south,east and west walls;oil was visible floating on the groundwater in the open excavation. The soil samples were forwarded to CET for analysis by ETPH;as indicated in the CET Analytical Report(CET 411090259)included with this report in Appendix C,ETPH was reported remaining in the soil near the water table above the Connecticut Department of Energy&Environmental Protection(CT-DEEP)standard of 500 parts per million. The soil sample collected from the post excavation area at RW2 indicated no ETPH present above the detection limit from that area. A 2"diameter injection lateral was installed in the RW1 excavation along with five surrounding injection points. The area was completely backfilled and brought to grade. t i Mrs.Nancy Clark CT-DEEP Case#2011-04450 August 23,2012 Page 3 of 4 One thousand four hundred and fifty(1,450)gallons of water was purged from RWland was disposed at United. Groundwater observed from within HPSA,HP6A,RW2,GP9 and GP 10 all remained clean and free of any nuisance characteristics of fuel oil impact. Oil sorbent wicks,which absorb(only)oil,were installed in RW 1,RW2,HPSA and HP6A. September 15`h—December 21s`, 2011 Enviroshield restored the floor while leaving access to well points for injection of BioSolve®(a biodegradable surfactant), purging and monitoring. During this span of time,Enviroshield staff utilized the injection well and points to flush BioSolve®through the surrounding soil to strip the residual fuel oil from the soil identified in the post excavation laboratory analysis. Enviroshield removed and disposed of 2,850 gallons of groundwater from RW 1 during this time span. Following the final purging event on December 21,2011,no free floating oil was observed in the recovery well RW I. No free floating oil or petroleum sheen was ever observed in any of the other wells installed inside of outside of the foundation footprint during the course of this project. Oil sorbent wicks were installed in RW 1 to collect any residual product,if present. A drum of sorbent waste material was transported to United for legal disposal;a copy of the Non-Hazardous Waste Manifest is included in Appendix B. Friday, February 17, 2012 All sorbent wicks were removed from the wells;there was no oil visible on any wick within any well. A sample of water was collected from RW1 and there was no nuisance characteristics within the water sample collected. Sorbent wicks were again placed within the wells. Thursday,May 17, 2012 All sorbent wicks were removed from the wells;again there was no oil visible on any wick within any well. A sample of water was collected from RWl and there was no nuisance characteristics within the water sample collected. Sorbent wicks were removed from all wells. Thursday, May 24, 2012 All wells were check for signs of oil impact or nuisance characteristics;there were no indications of fuel oil impact to the groundwater inside or outside of the footprint of the subject site foundation. All wells filled and legally abandoned and the floor was restored with cement. � 1 Mrs.Nancy Clark CT-DEEP Case#2011-04450 August 23,2012 Page 4 of 4 Conclusions: 1) Based on laboratory analytical reports and visual observations,fuel oil had impacted soil and groundwater under the foundation of the subject residence. 2) Based on the remedial efforts made which included the excavation and legal disposal of 7.41 tons of sub-slab soil and 4,765 gallons of groundwater from under the foundation, it appears that the accessible oil impacted soil has been removed. Further,no remaining free phase fuel oil or nuisance characteristics were present on the groundwater table within any well on the subject site. it appears that all free phase oil has been removed from the groundwater under the basement flooring. 3) No impact by fuel oil constituents to soil or groundwater were ever observed(by visual observation)in the two well points located outside of the foundation. 4) Consistent with the November 8,2011 CT-DEEP Guidance for Residential Underground Home Heating oil Tank Releases,soil has been removed to the maximum extent practical,no objectionable odors occur within the subject residence and no floating product remains on the groundwater table. Subsequently,no further work is required or recommended. 5) All work performed as part of this project as described herein,was completed or overseen by Enviroshield,Inc.,a State of Connecticut licensed spill cleanup contractor according to the guidelines established by the CT-DEEP. No other statement of environmental conditions regarding the subject site are made or intended other than what is specifically mentioned and addressed within this report.
