Captain Bengston stated that the fire was confined mostly to the second floor bedroom located on the East side of the house at the top of the stairs. The fire did extend into the hallway area with minor damage. Captain Bengston stated that when his crew arrived they forced entry to the front door and went up the stairs to extinguish the fire. The interior attack crew was headed by Lieutenant Dan Katzbek. The Fire Department extinguished the fire with minor extension into the hallway. The Fire Department broke the 2 bedroom windows in the North bedroom located on the second floor. No injuries were reported. After meeting with Captain Bengston, I talked with Lieutenant Katzbek. Lieutenant Katzbek stated that when he arrived all occupants were outside the house. The crew had to force the front door and stretch a line upstairs to the East bedroom at the top of the stairs. The crew ventilated the bedroom at the top of the stairs by pushing an air conditioning unit out of the window. The fire was mostly confined to the bedroom. Occupant(s) Interview(s) I interviewed Amanda LaChance. She stated that she was down in the kitchen on the first floor when she heard the second floor smoke alarm going off. Amanda went to see why the smoke alarm was going off. She stated that when she looked upstairs she saw smoke coming out of the sides and bottom of the door to her brother's room (East Bedroom). Amanda stated she ran upstairs to see if her brother was ok. When she opened the door, she saw most of the bed and lot of the wall was ignited. She and her mom were yelling for her brother who eventually came out of the bathroom located to the South of the bedroom. The father came up from the lower level and told everyone to get out. She stated that Cameron and her father went to the garage and got a fire extinguisher. Amanda and her mother got the dogs out of the house and called 911. Statement attached to report. Cameron stated that he had just come home. He went up to his room and put his hat, some keys, and an iPod on a small table next to the bed by the dresser. He stated that he turned on the window air conditioning unit and grabbed a towel to take a shower. Cameron stated that while he was in the shower he heard his family yelling for him. He got out of the shower and helped his father with the fire extinguisher. He stated that they tried to put the flames out with the extinguisher and a couple of small buckets of water, but the flames were too much. He then exited the house. I interviewed Kathleen LaChance (mother) next. Kathleen stated that she was in the kitchen with her daughter Amanda. Her son Cameron came through the door and went to his room and then went to take a shower. She stated that her husband Robert was in the lower level watching TV. Kathleen stated that about 10 minutes later Amanda asked her what was burning and Amanda went to investigate. When Amanda opened the door to the bedroom, she saw the bed on fire. They found Cameron and gathered the dogs. She stated that her husband tried a fire extinguisher, but that did not work. Statement attached to report. I interviewed Robert LaChance (father) next. Robert stated that he was in the lower level watching TV when he heard the upstairs smoke alarm sounding. By the time he got upstairs, Amanda had opened the door to Cameron's room. He stated that he saw flames already up to the ceiling from the bed. Robert got his son out of the bathroom and went to the garage to get a fire extinguisher. He stated that when he got back with the extinguisher he could make it up only 1 or 2 steps because the fire was coming out of the room into the hallway. He emptied the extinguisher and the flames went down. Robert and Cameron tried to put some water on the fire, but it had reignited and the whole room was burning. He and Cameron left the house. After these initial interviews were conducted, a systematic examination of the scene was conducted starting with the exterior of the house. Exterior Examination: Exterior examination began on the West side which is the front of the house. The front of the house has minor smoke damage above the front door (see photo 1). The front door was examined and revealed signs of forced entry by the Fire Department. The North side of the house revealed minor smoke damage above the gable vent and a window from the upper floor. The electrical service is located on this side of the house and revealed no damage (see photo 3 & 4). The East side revealed major damage to the upper window in the middle of the house with extension to the soffit area (see photo 5). A pile of room contents was located on the ground with the air conditioning unit below this area (see photo 6). The South side revealed only minor smoke damage to the gable vent (see photo 7). Interior Examination: Following the exterior examination, a systematic examination of the interior of the structure was started. Starting from the least damage to the most, I examined the garage first. The garage showed no damage (see photo 8). 1 entered the structure through the kitchen which is on the main floor. The kitchen revealed minor smoke staining layer down about a foot from the ceiling. Food was still on the stove and no items were out of place (see photo 9). 1 went to the lower level of the house. This level had a furnace room and den. The furnace room was examined and revealed no damage and the furnace was in good shape (see photo 10). The den revealed no damage (see photo 11). The electrical panel was found in this room on the North wall (see photo 12). The circuits were examined and one circuit breaker was tripped. This circuit breaker, number 16, was tripped which serviced the upstairs and bathrooms (see photos 13). 1 went back upstairs and went to the main level and examined the living room. The living room sustained major smoke staining with a smoke layer about 2 feet down from the ceiling. All furniture was in place (see photo 14). From the living room on the main floor, a set of stairs leads to the 2nd floor. The 2nd floor contains 3 bedrooms. One directly at the top of the stairs, which was Cameron's room. The North bedroom which was Amanda's room. The South Bedroom which was the parent's room. In between the parent's room and Cameron's room was a bathroom. The stairway leading up to the 2nd floor revealed smoke staining about a foot down from the ceiling (see photo 15). The hallway was examined next which revealed heavy smoke and heat damage around Cameron's room. The smoke detector was missing (see photo 16, 17 & 18). Heat damage moving out from Cameron's room can be seen by a line of demarcation upward from the entry door about 3 feet down from the top of the door towards the North bedroom (see photo 19). Amanda's bedroom (North) was examined next. The room sustained minor heat damage and extensive smoke damage. The North window sustained the most damage with heavy smoke staining. The curtain was hanging down but intact. All items in the room were in place but smoke stained. The smoke layer was about 4 feet down from the ceiling (see photos 20, 21, & 22). Moving down the hall the bathroom was located on the other side of Cameron's room. The bathroom sustained minor damage (see photo 23). Moving down the hall the parent's room was next. The parent's room was located on the South side. The room entry door showed a smoke line of demarcation moving from the hallway into the bedroom in an upward motion from about 2 feet down the door (see photo 24). The rest of the room revealed minor smoke staining with all windows in tact (see photos 25, 26 & 27). At this time, I moved to Cameron's room. Cameron's bedroom entry was examined. The entry door frame received heavy heat damage on the top half. The door itself was located inside the room. The top half of the door was missing (see photos 28). As you walked in the room a dresser was located on the South side. The dresser sustained more damage on the East side by the bed. Items were still intact on top of the dresser(see photo 29). The West side of the room contained a computer area and dresser. All items located on top of the computer table sustained heat damage. The computer and electrical outlet located under the table were in tact (see photos 30, 31, & 32). The light switch located on the wall by the entry door sustained heat damaged (see photo 33). The North area contained a dresser and closet. The dresser was intact. The closet sustained major heat damage with the top half of the door missing (see photos 34 & 35). The East wall sustained the most damage. The window is missing and half the window trim is missing. The sheetrock is mostly missing. The fire extended beyond the sheetrock and extended to the roofing plywood just over the window area (see photos 36 & 37). The ceiling sheetrock was on the floor. The roof rafters were in good shape and appeared to sustain no damage (see photo 38). The floor was examined. The floor contained numerous items and clothing in a thick layer. The bed was found in front of the East window (see photo 39 & 40). With this room having the greatest damage and witness statements to collaborate this, Cameron's room is the room of origin. Room of Origin Examination: A systematic delayering of the room commenced. The room contained a lot of items and clothing on the floor. The floor and bed area was cleared. Next to the dresser by the bed was located a small coffee table. This table was placed in its STATUS OF CASE CODE PAGE NUMBER 1.ACTIVE A.Accidental C. Providential 2.CLOSED B. The legs by the bed were missing. The underside of the table was heat stained. Items were still intact on the top (see photos 41 &42). An electrical duplex outlet was located on the South wall in this area. The outlet contained a 3 way extension and a transformer. The 3 way extension and transformer were examined and were intact. The outlet was examined and found to be in good condition. The wiring behind the outlet was in good condition (see photos 43, 44, 45). The area under window and bed was examined next. This area had the lowest burning. The area contained an outlet directly below the window. This duplex outlet was examined. The outlet contained the remnants of a bonding attachment from a three to two prong adapter. Only one prong of the plug was in the outlet (see photo 46). The bonding wire to the mounting nail was in good shape. The electrical wire supplying the outlet was in good shape (see photo 47). The outlet cover was taken off and the outlet received heavy heat damage to the exposed side (see photo 48). The wiring in back of the outlet was in tact and no beading was seen (see photo 49). The mattress and box spring had no material left on it. The bed frame was damaged more on the side of the East wall (see photos 50 & 51). The air conditioning unit that was on the ground in the back yard was examined next. The wiring for the plug only had about 6 inches left with no plastic covering. The wiring did not have any beading on it (see photos 52 & 53). Follow-up Interview: At this time I brought Amanda back into the room and asked her where she saw the fire when she first came into the room. She pointed to the area of the bed right under the window. Area of Origin: At this time, based on the physical evidence and witness statements the area of origin was determined to be in the area of the Cameron's room under the window. The investigation concluded at 01:53 hours on 03/21/12. Conclusion: The fire that occurred within the structure identified as 44 peach Tree Lane in South Windsor, CT on 03/20/12 at 20:41 hours is determined as follows: The examination of the fire scene and based on the above described burn patterns, lines of demarcation, examination of the physical evidence at the scene, as well as witness statements it has been determined that this structure fire origin and cause is undetermined.
