Investigation and Remarks:
7/18/17 10:25am — Edna called, talked with HO. She had her nails done on 7/12/17 by Lisa. She
has since developed what appears to be an infection in her hands. The skin around her nails is
damaged and raw. She is visiting the doctor 7/18/17. HO to make site visit to evaluate cleanliness
and practices between customers.
3:40pm —Visited establishment. Staff unwilling to discuss anything, kept saying "No manager".
Five females working at time of visit. There was a very heavy odor of acetone/chemicals during
visit. Woman reported manager's name is "Snow" and directed HO to the phone number on the wall
for the Farmington location; 860-409-9200.
7/19/17 2:55pm — HO called the Farmington number. Spoke with manager, Amy. Amy would like to
speak with complainant about specifics from visit. Inspection scheduled for 7/20/17 at12pm to verify
cleaning and sanitizing procedures.
3:1 pm — HO called Edna to discuss Amy's request for a phone call about the situation. Also
discussed the upcoming scheduled site visit.
7/20/17 12pm - At establishment with Amy Pham. The foot baths are run after each client with
bleach. Suggested not doing additional rinse with Fabuloso. Amy requested the EPA
recommendations for foot baths be sent to her at the Farmington location. Items found at pedicure
stations to be reused (files/buffs) soaked in Barbicide between uses. Labels are needed at all
bottles in establishment; only a few labeled properly. Suggested single use disinfecting wipes in
baths after each client instead of kitchen sponges being used. The hand/nail stations cleaned daily.
No files reused between customers. They have onsite laundry. UV lightbox lined with towels;
recommended removing so UV light would better penetrate items being sanitized. Overall,
establishment is clean. A few adjustments to their practices needed, but no major violations noted.
