Subject: Qualified Food Operator Requirements
Dear Mr.Mazzarella:
On March 8,2017, I conducted a routine inspection of the above referenced food service establishment. At that time it
was observed that you and no one under your employment that had a Qualified Food Operator Certificate(QFO). Per
the regulations, Connecticut Public Health Code 19-13-b42(s)(7) "a replacement qualified food operator shall be
employed within sixty(60)days from the date of termination or transfer of the qualified food operator".
You and/or one of your full-time managers who meets the criteria for a QFO(see attached) must complete the
course and receive a passing score on the exam by Mav 9,2017.You could also hire someone full time who has a
QFO certificate at your facility. Proof of certification must be provided to the health department. Failure to do so will
result in further administrative action.
Please contact me if you have any questions or need any additional information at 860-337-6182.
Sincerely,
Diane L.Collelo, REHS/RS
