On December 1, 2016, this department received a B100a application for the proposed
addition on your property, 2039 John Fitch Boulevard, South Windsor.
This approval is granted under Section 19-13-B100a of the Connecticut Public Health Code with
the following conditions.
1. The inspection performed on 9/23/16 by Skip's Wastewater indicates that a replacement
septic tank is needed. A repair permit, plan, and fee of$10 must be submitted to this office.
Upon approval, the required inspections will be performed with the installer.
2. The replacement tank and any future repairs of the septic system on this property will be
calculated for a 4-bedroom home; unless more bedrooms are added in the future.
3. At the time of the foundation excavation for the addition, soil testining will be conducted
because there is no existing information for this property.
4. As discussed with Diane Collelo on December 1, 2016, this addition will be in conformance
with the floor plans submitted and no additional bedrooms will be constructed.
Approval is given with the understanding that proper care and maintenance of your existing septic
system be taken. In the event that the existing subsurface sewage disposal system fails, you will
be required to install a replacement system.
Be advised, that you must obtain all appropriate approvals from other town departments prior
to commencing work on this project.
