Dear Ms.Temple:
On August 29, 2016 this department received an updated food service plan NORR Architects,
dated 6/8/16)for the above referenced food service establishment.
The plan was approved with the following conditions:
1. Choking poster is to be posted in an area visible to employees.Handwashing procedure
signs to be posted at each handwashing sink.
2.No smoking sign needed at entrances to Building. -
3. Qualified Food Operator and Designated Alternate Form to be submitted to health
department(see attached).
