Dear Customer:
Pursuant to Section 19-13-B 102(f)(3) of the Public Health Code, a cross connection
inspection was performed by our company at 1590 John Fitch Blvd-South Windsor on 02/13/09.
Our inspector identified the need for the installation of a Double check backflow preventer
assembly, as indicated on the attached inspection sheet. These installations are necessary for
compliance with State Public Health Code Regulations.(see attached)
Please notify our office within 90 days of this notice to schedule an inspection of the
installations. Upon installation, a successful test of the devices must be completed by a certified
tester and the result sent to our office for addition to our annual report to the State Health
Department.
Should you fail to perform these installations within the allocated time schedule,I will
request assistance from the local and state Health Departments for enforcement of the Public
Health Code.
