Carey 48 East Dudley Town Road Bloomfield, Connecticut 06002 Dear Mr. Carey: This correspondence is to confirm conversations during the morning hours of September 21, 1995 with Mr. Carey, Mr. Vitti and associates, including the owners of Between Rounds . Upon arrival at the site, I observed two septic tanks and several leaching galleries which had been installed previously. These appeared to be placed too low into the existing grade, and too close to the existing water table. At that time I was also given a copy of plans for this installation with additions date of 6/18/95 and revision dates of 7/27/95, 9/18/95 and 9/20/95. The only plan which was approved by this department and the State Health Department was the one dated revision 7/27/95 . ( It must be noted here that this approval was based upon the inclusion of two omissions as described in correspondence to Joseph Puiia dated 8/4/95, a copy of which is inclosed. ) After discussion with contractual personnel on the site it was determined that the system was thought to have been installed approximately 18 inches above the water table as indicated from soil testing conducted on March 13, 1995 . This installation apparently was based on eighteen inches above the existing water table. As you are aware the code states that the system be installed eighteen inches above the maximum water table as indicated by soil mottling. Although no soil mottling was observed on 3/13/95 it was reasonably felt that the approved installation with all inverts at elevation 96 . 0 would provide code compliance with a maximum water table. September 22 , 1995 Page 2 Based on observations made at the bottom of the installed gally trench and the known observed water table, it has been determined that the gallery units which have been installed be removed and that all units be installed as per the approval of revised date of 7/27/95 including the referred letter. Invert elevations will be no lower than 96 . 0 . The septic tanks- must be raised accordingly to accommodate this change. Additionally, new plans must be drawn to reflect these changes including tank locations, all elevations, location of sewer lines (with invert elevations) . If the proposed bathrooms at the rear of the building are to be served by a separate sewer pipe that must also be on the plan. In summation, anything pertinent to the installation must be on the next submitted plan. Any changes, additions, etc. will require the submission of another plan and approval. Finally, the "as built" must indicate conformance as reasonably as possible to the approved plan in order for final approval to be given by this department. Very truly yours, Robert Dept la, R. S. South Windsor Health Department 1540 Sullivan Avenue South Windsor, CT 06074 Re: Approval of Plans for Between Rounds, Inc., 19 John Fitch Boulevard, South Windsor Dear M eptula: We have reviewed plans for the relocated subsurface sewage disposal system for the "Between Rounds" Bakery and Deli at 19 John Fitch Boulevard in South Windsor. Plans were prepared by the consulting firm of Mattson Associates, Armand Fisher, P.E. and submitted to this office for review pursuant to Section 19-13-B103d(e) of the Public Health Code. The plans bare a revision date of June 18, 1995. These plans were found to be generally satisfactory and in accordance with the requirements of the code. We recommend that they be APPROVED by your office with the following stipulations and modifications: 1. The maximum daily design rate for this building has been set at 1300 gallons per day (GPD) . Therefore, once occupied, the maximum average daily discharge shall not exceed 867 GPD over any 30 day period. 2. We recommend that all leaching galleries be set level, with the bottoms set at elevation 92.83 (the inverts at 96.00) . This will allow equal distribution into each gallery row through a splitter box. 3. First septic tank should be rotated 90 degrees so that sewer line could be installed in a relatively straight line, avoiding the necessity of cleanouts to grade. 4. All gallery units shall be H-20 loading strength. Phone: Z Telephone Device for the Deaf (203) 566-1279 150 Washington Street — Hartford, CT 06106 An Equal Opportunity Employer 2 - 5. CONN. 06074 R.co' AREA CODE 203/644-2511 December 20, 1990 Mr. Nicola Modugno 278 Main Street South Windsor, Connecticut Dear Mr. Modugno: This letter will serve to confirm conversations with yourself and your installer pertaining to the installation of the septic tank on your property located at 19 John Fitch Boulevard,- South Windsor. Since this installation is to replace an existing tank, variances can be granted to utilize the same area. However, if and when the existing leaching area fails, the presently used area on the south side of the building must be abandoned and the more spacious area on the north side of the building will have to .be utilized.
