Doolittle, Town Engineer CC: Fred Shaw FROM: Joseph J. Perna, Project Engineer p DATE: November 26, 2003 RE: Aunti Dawn's Bake Shoppe 601B Sullivan Avenue On November 25, 2003, I observed the locations of the existing sewer connections for 601A and 601B Sullivan Avenue. Mr. Jim Throwe was on site and he had exposed the sewer connections at the existing house (#601A) and at the existing building (#601B). Rick Martel of Design Professionals was also on site to document depths and locations of the laterals. The sewer lateral for #601A is located on the west side of the house and was approximately 5 feet in depth. I checked with the Tax Collectors office and it appears that the house has been connected since 1977 although I could not find any record of a permit being issued or an inspection taking place in our files. The sewer lateral was constructed of 6 inch PVC with o-ring joints (gray pipe) and consisted of a series of elbows connected to one straight section of pipe that dropped approximately 3 to 4 feet to a "T-wye"connection. The sewer lateral for #601B is located on the southwest side of the existing building and was approximately 1.5 feet in depth. Jim Throwe had apparently installed this lateral about two years ago without a permit. When I mentioned that a permit should have been taken out, Jim stated that he had asked John Collins of the Building Department and he said that a permit was not required. In any event, Jim had installed the "T-wye" connection at the house and ran the sewer lateral around the back of the house to the existing building. The connection at the building consisted of a section of 4 inch PVC connected to a 6 inch PVC pipe. In both trenches, I did not observe any stone bedding and the pipe was backfilled with native material which had a sandy consistency. The proposed manhole is in the yard area adjacent to the house for access to effluent for testing and had a depth of 4'. The proposal from Mr.Throwe showed a system with an inside diameter of 2'. Joe Perna and myself reviewed the information and determined that the system would function adequately with a change to an inside diameter of 4'. 1 informed Mr.Throwe of our approval of the C48WF system with a 48"inside diameter submitted.
