PROJECT: Savings Bank of Rockville REPORT NO. : 041
INSPECTOR: Mark Richardson DATE: 06/05/00
SUBJECT: Shop Inspection RE: BOCA PAGE: 1 of 5
Section 1705
A shop inspection was performed on this date at the fabrication
plant, Leo Chasse, Inc . , Manchester, NH, for the purpose of
ascertaining that applicable codes and specifications were being
implemented during fabrication for the project . The following
specifications and codes are being applied on this project :
Project Specifications Division 5-Metals (Structural Steel) ; AISC
Manual of Steel Construction; The American Welding Society Code
D1 . 1 Edition for Structural Welding; ASTM Standards for Metals
Conformance and Non-Destructive Testing; and the (SSPC) Steel
Structures Painting Council, Vols . 1 and 2 for Steel Cleaning and
Painting.
The writer was at the above-stated facility to performed a Part A
Shop Inspection and to monitor shop production, i .e. dimensional
accuracy and welding. The writer met with Mr. Matt LaVoie, Vice
President and Mr. Howe, Manager. Mr. LaVoie and the writer then
met with the shop foreman in the shop. The writer received a set
of prints (shop drawings) from the shop foreman. The writer then
started to go over the questions for the shop evaluation report
which is attached.
The shop is cluttered and poorly lit . Equipment is greasy and
crowded with material, i .e. saw, angle brake both of which were
not in use during this visit . Assorted power and hand tools were
in fair to poor condition.
Dimensional accuracy and welds were checked using shop drawings
(not approved) , fillet weld gauges and AWS D1 . 1, Table 6 . 1 for
visual inspection acceptance criteria.
Shop welders were using Lincoln Ideal Arc DC-400 power source,
runs at 200 DC amps and 30 volts; wire feeder is LN-7, 18-20 ipm;
shielded gas is 75% argon and 25% carbon dioxide and runs at 40
cfm which leaves an excessive amount of splatter.
Test reports may not be reproduced except in full with approval of IMTL. All results relate only to the items tested.
Test reports must not be used by client to claim product endorsement by NVLAP or any agency of the US Government.

The writer placed a call to Mr. Dawson of Russell Dawson
Engineering to discuss missing welds on Piecemark 5B12 . Weld
called for was 2" every 6" on SB12 and not in accordance with AWS
D1 . 1, Section S-5 . 13 for compliance with design, size, length and
location. Mr. Dawson was also informed of added welds on
piecemarks and splatter. Mr. Dawson stated that additional welds
were acceptable and welds off location are satisfactory as per a
telephone conversation on June 5, 2000 .
The writer also received copies of welders qualification records.

The writer was at the above-stated facility to monitor ongoing
work. The office was informed of the writer' s presence on site.
Upon walking into the shop the writer noticed that splatter on
columns waiting to be loaded was not cleared and members were
painted. Undersized welds were not repaired. The writer asked
the shop foreman if welders have welding gauges . The foreman ask
if they needed them? The writer used the gauge to demonstrate to
the foreman that columns being worked on have undersized welds
according to shop drawings. The foreman was informed that
splatter and undersized welds must be repaired.
Beam 4B3 was detailed to have 2-1/2x2-1/2 angles welded to
underside of flange. The Foreman welded them to the web and
underside of flange. Dimensional accuracy and welds were
inspected during the course of the day using fillet weld gauges,

PROJECT: Savings Bank of Rockville REPORT NO. : 051
INSPECTOR: Mike Brault, CWI 91100111 DATE: 07/07/00
SUBJECT: Field Verification Inspection PAGE: 1 OF 3
A field inspection was performed at the above-referenced project
today. The purpose of this inspection was to verify that
completed work was performed utilizing the engineer-approved
drawings, the project specifications, and the American Welding
Society Structural Welding Code D1.1.
Field verification inspections were performed on structural steel
framing on the Second Floor as follows :
Column plumbness verified for conformance with AISC 1 :500
tolerance on columns from the First Floor/slab on grade to Second
Floor framing from B to M Lines, 1 to 14 . Columns were found
plumb within AISC tolerance.
Column base anchor bolts were surveyed for completion and snug
tight anchor bolt nuts. Several locations were found loose or
incomplete and insufficient thread projection, i.e. short anchor
bolts were noted at E-8 (11C8) and K-11 (16C2) . Both require
engineer-approved field repair.
Visual weld inspections of field welded floor joists on the
Second Floor framing and bridging welds were found acceptable at
this time.
The writer was informed today that while tensioning the Second
Floor framing bolted field connections, i.e. double connection
angle knife connections, about 40% of ASTM A325 high strength
tension control bolts were stripping out. This condition was
occurring in bolts with lengths of 2-1/2" and 2-1/4" . The writer
witnessed several field connections being tensioned and noted the
same results. The erector used three (3) separate tension
control devices (TC guns) and the results were the same. Mr.
Maurice Grenier, Steel Fab' s QC Manager, arrived with their
Skidmore Whilhelm device with results detailed on the attached
sheet .

Steel Fab has contacted the manufacturer in regards to this
condition as well as Mr. Bob Dawson of Russell Dawson Engineers
who was informed of results of bolted connections.
This task was performed by Steel Fab and replacement bolts were
fully tensioned and found satisfactory upon visual inspections at
knife connections on a majority of field connections .

The writer generated an ongoing steel items list which details
incomplete and remedial areas which need to be addressed by Steel
Fab.
At Third Floor field bolted connections the writer noted today
that the same situation is occurring with ASTM A325 tension
control bolts. During tensioning by the erector bolts are
continuing to strip prior to the tension control splines snapping
off. Bolts at this elevation will also have to be replaced by
Steel Fab.

A field inspection was performed at the above-referenced project
today. The purpose of this inspection was to verify that
completed work was performed utilizing the engineer-approved
drawings, the project specifications, and the American Welding
Society Structural Welding Code D1.1.
Field verification inspections were performed on structural steel
framing on the Second Floor as follows:
The writer arrived on site to obtain a resolution to the ASTM
A325 high strength tension control bolt issue with 2-1/4" and 2-
1/2" 3/4" bolts, Lot Nos. 981210 and 81119008 noted in field
report dated July 7, 2000 .
Mr. Bob Dawson of Russell & Dawson Engineers and Steel Fab' s QC
Manager, Mr. Maurice Grenier, reached a resolution on field bolts
per our verbal conversations between Mr. Dawson and Mr. Grenier.
Mr. Dawson felt that all bolts that had stripped out during
tensioning should be removed and replaced with new A325 bolts .
This task was performed by Steel Fab and replacement bolts were
fully tensioned and found satisfactory upon visual inspections at
knife connections on a majority of field connections .

Seated connections still require bolts to be installed throughout
the Second Floor framing. The writer informed the erector of
this.
Beam seat stabilizer angles, i .e. roll clips, were being welded
at the Second Floor framing today but were not all completed.
Welding on the Second Floor metal deck was also in progress
today. 26-gauge form deck was being puddle welded to substrate
members with the use of welding washers as required. Spacing was
being adhered to for welding patterns .
Connection of beams, Beams 4B9 to 5B11 just east of K-11, Second
and Third Floors, and at Beams 6B8 to 3B2, Second and Third
Floors were f ield-welded by the erector due to bQ►''', %ci %
misalignment. Steel Fab is to produce field-weld sketGN6 FTC

Society Structural Welding Code D1.1.
Field verification and visual weld inspections were performed on
column shop-deposited welds found unacceptable and undersized
during shop inspections at Leo Chase Fabrication, Manchester, NH.
Steel Fab, Inc. performed remedial welding in the field to comply
with drawing, specifically weld sizes at the column to base plate
connection.
The following base plates were field repaired by Steel Fab and
reinspected and found acceptable by the writer: Column 14C2,
15C1, 15C5 and 16C2 .
Column base anchor bolt nuts were verified as being installed and
snug-tight at this time except for the following locations:
Column E-8 (118C) and K-11 (16C2) had insufficient thread
projection through plates. Steel Fab has welded anchor
stubs/studs to base plates.

A field inspection was performed at the above-referenced project
today. The purpose of this inspection was to verify that
completed work was performed utilizing the engineer-approved
drawings, the project specifications, and the American Welding
Society Structural Welding Code D1.1.
Field verification inspections were performed- on structural steel
framing from the Second Level up to the Third Floor framing as
follows :
Column plumbness was verified for conformance with AISC 1 :500
tolerance from the Second Floor to the Third Floor Framing.
Columns were found to be plumb within the above tolerance.
Visual weld inspections were performed on field welded floor joist
to support beams, bridging, seat beam stabilizer angles and roll
clips. Welds were fond to be satisfactory at this time.
ASTM A325 field bolted connections at the Third Floor framing were
visually surveyed for proper installation and tightening. As
noted by the writer in field report dated July 11, 2000, high-
strength bolts are stripping out during tensioning. Steel Fab is
aware that bolts are stripped and will have to be removed and
replaced per Mr. Dawson's instructions.
Several of the connections, i.e. knife connections, seated beam
bolts and beam to beam connections, were noted as loose and/or
incomplete today. The erector was made aware of the locations to
be completed and will rework bolted connections on Tuesday, July
18, 2000.
