Schedule of Special Inspections: Prepared by: lKenneth Alley PE
Dated: 6/7/02
A brief meeting was held with the contractor's project superintendent concerning the requi:,n-rents of the
program a,% shown in the schedule. BBE was provided with a copy of the schedule and has w4:-rked with the
Special Inspection "'system" before.
We examined each section of the schedule and Y explained what inspections were needed to cotr, yly and enable
us to prepare the Final Deport of Special Inspections that will be needed before the Building Ofl icial will issue
a Certificate of Occupancy.
The following issues could easily present problems in the future if not handled correctly.
All inspection services are to be provided by STL and the scope of the inspections is cle 11-ly defined in
the schedule. The soils, concrete, structural steel and masonry inspection program:, are all quite
conventional,
• An inspection of the subgrade for suitability prior to the placement of the foundati,.Nns leas been
accomplished and further reviews will occur in the future.
A review the concrete nix design is required. We would appreciate copies of the Pape'-rvork so that
this review can be accomplished.
• A review the structural steel mill certificates is required, We would be grateful .for ;spies of the
paperwork so that this review can be accomplished.
— • A re-view of the material certifications for the: masonry materials is required. We woi.I appreciate
copies of the paperwork so that this review cin be accomplished.
During the meeting Mr. Abbas agreed that l could perform the Inspection of the subgrade inst lLd of haying
Dr Welti make a special trip and that was duly accomplished. He also agreed that our moss experienced
masonry inspector, who is not a Professional Engineer, has sufficient expertise to adequate'..,r inspect the
masonry construction, Both of these alterations to the;written scope of the Special Inspection i; rogram have
been incorporated into the program unless we receive notification to the contrary.

This review covers the period from May 22 to June 14, 2002.
The first inspection of the soils exposed .for .foundation bearing was conducted on May 23. Tile soils were
found to be as anticipated by Dr Welti's report and the contractor was placing the gravel or the exposed
soil surface as required.
During the period covered by this rerAew we have conducted inspections of reinforcing ant: concrete on
May 28, 29, 30, June 3, 4, 6, 1.0, 1.)., 1.2, 13 and 14. Reinforcing alone was inspected on June .
Air contents of 5% and higher have been found on several occasions. However, providins, the ultimate
compressive strength of the samples reaches the required level, Z do not propose to treat tl'irse values as
"discrepancies." If the Architect or Structural Engineer wish me to adopt a different approal.Oh, please let
me know.
Cylinder samples of the concrete have been prepared transported stored and tested as re,:,uired by the
project documentation. During the period covered by this review compression tests have be,.-n performed
on six such samples, all of which were seven days old at the time of test. Each sample! :achieved an
ultimate compressive stress capacity greater than that recommended (65% of the req-ured 28-day
strength).
All of the reinforcing arrangements observed during these inspections was in accordaa,Ge with the
documentation referenced on the reports.
Soil placement in have been performed on June 12, 13 and 14. All soil densities recorded on
those reports met or exceeded the minimum specified in the construction documentation.
Outstanding Information Required to Complete the S-;=ial Inspection Reviews
The concrete mix design.
The masonry material certification and mix design will be needed in the near future
The mill certificates for the structural steel will be needed in the near future.
Discrepancies Requiring Re-inspection
None at this time

As required by the Schedule of Special Inspections%ve have reviewed copies of the concrete rit.ix designs for
the 3000-psi concrete. One of the designs was for concrete with fibermesh but without an air ent a.ini.ng additive
and the other was for concrete with an air-entraininn, additive.
The concrete supplier also provided us with sieve analysis data that confirms the aggregate's c!::rnpliance with
ASTM C 33 as required by the project construction documentation.
The mix design for concrete with an air-entraining.additive complies;with the project document tt;on in almost
all significant respects and has been accepted by the project architect. 'The deviations fro!n the project
documentation can be considered trivial and will no he considered as"relectable discrepancies' unles,,we are

The copy of the lnix design that we have for concrete without an air-entraining additive core plies with the
project documentation in almost all significant respects but hm not been through the approva process. The
deviations from the project documentation can reasonably be considered to be trivial and will wl therefore be
considered as "rejectable discrepancies" unless we arP notified to the contrary,

Schedule of Special Inspections- Prepared by: Kenneth Alley BE
Dated: 6I7JO2
This review covers the period from June 17 to July 19, 2002.
ALn inspection ofthe soils exposed for foundation bearing was conducted on June 18. The soik; were found
to be as anticipated by Dr Welti's report and the contractor was placing the gravel on the exposed soil
sui face as r equh ed.
During the period covered by this review we have csonducted inspections of reinfomiing an('; concrete on
June 17, 181 191 20, 21, 247 July IS, and 19. Concrete alone was inspected on July 19
Air contents of 5% and higher have been found can only one occasion during this pericii:i. However,
providing the ultimate compressive strength of the Nemples reaches the required level, I do n:--t propose to
treat this value as a "discrepancies,'' 1f the A..rchitect or Structural Engineer wish me to ado':',t a different
approach, please let me know.
Cylinder samples of the concrete have been prepared transported stored and tested as rec, aired by the
project documentation. During the period covered by this review compression tests have be,,n performed
on 66 such samples. Each samples except for two made on .Tune 1.1 achieved an ultimate compressive
stress capacity Greater than that required by the construction documentation. The last san-ple available
from that group is being held for testing at 56 days. Samples made on June 21 and 24 failed i--I achieve the
rernmircenned compressive gtrecc capacity when tested at %even days old. .I do not propose' to treat this
issue as a "discrepancies." If the Architect or Structural Engineer wish me to adopt a differ+!131 approach,
please let me know.
All of the reinforcing arrangements observed r ur:ng thiase. inspenrinns war, in ancorda.r :e with the
documentation referenced on the reports.
Soil placement inspections have been performed on June 17, 18, 19, 20, 24, 24, =6, 27, July- , 15, 17 and
18. All soil densities recorded on those reports met or exceeded the minimum sper.;ified in the .7orstruc;tion
documentation.
Inspection of the material and workmanship being provided to the erection of the structunii. steel frame
urere performed on June 20", July 10 and 18. Some minor and easily nnrrpntnblt,. nnhimin vprketAlity i.volec

This review covers the period from July 22 to AuSUSt I6, 20021
The inspection program for the soils exposed for foundation bearing was is complete. The :oils have been
found to be as anticipated by Dr Welti's report and the contractor was placing the gravel i-n the exposed
soil surface as required,
During the period covered by this review we have conducted inspections of reinforcing i:,n.July 30 and
concrete on July 24, 30 and August 6, 2002
Air contents of 1-2% have been found, consistent with the requitement that concrete without an air
entraining anent be used for the slab on grade.
Cylinder samples of the concrete have been prepared transported stored and tested as r,quired by the
project documentation, During the period covered by this review compression tests have b:,en performed
on 46 such samples. Each sample achieved an ultimate compressive stress capacity grey:liter than that
required by the construction documentation at. 28 days or the recommended ultimate compr,.,ssiVe strength
capacity for those samples tested at 7 days, The last sample available fro.tal the group rnad,;: on June 11,
2002 was tested at 56 days and achieved an ultimate compressive stress capacity of 2300 psi
All of the reinforcing arrangements observed during these inspections was in accoi-&-.1ce with the
documentation referenced on the reports.
The soil placement inspection portion of the Special Inspection proLrarn was completed on ,Xugust 1, All
soil densities recorded have met or exceeded the min;rnum specified in the construction docun-:entation.
Inspection of the material and workmanship being provided to the erection of the structun; I steel frame
were performed on July 23, 26 30, August 5, 8 and 15. The undersized welds connecting th roof trusses
that were noted on July 18 were corrected, re-inspe,2,ted and accepted on July 23. On Augur,: S the report
notes that:
1) Kicker angles at line 6 have been installed 1 ba.y south of specified location at every ;.ondition (14
locations) due to fabrication error.
2) Panel paint reinforcing, not installed as per drawing JE-1 of 3.

Schedule of Special Inspections: Prepared by: Kenneth,AUcy PE
Dated: 6/7/02
As required by the Schedule of Special Inspection. referenced above we have visited the j i.ibsite to
review the bracing, stiffening joint details and general fit up.
I reviewed the completed frame and(.discussed the inspection program with the field inspector in Charge.
Ha cu,*nied my observations that the geancral quality of the work was as required by the cc:-,tstruction
documentation. However he brought to my attentioa the fact that the bottom chord extonsioi:ia for the
open web joists at the west elevation are to be corrected and the joist reinforcing between Sr:ds 1 and to
and F and C is currently incomplete.

An authorized representative of this firth was dispatched to the above project site to observe tl'is assessment
of the strength of in-place concrete using a Windsor Probe System(manufactured by James'.I;ixsfisuments)t
with the following results. The system oo plies with ASTM C803, and the results were i-iLtexpreted in
accordance with ASTM C670.
This assessment was conducteA by Don Penepent of Tilcon, the concrete suppliers, in resjionse to low
concrete compression test results for the samples taken the day these components were pour,;,,:d.
The locations that were tested were those from which the concrete samples made on June 11 were taken.
Specifically they were the frost wall on GbA Line to Fb-5.6 and the footing from B-1 to B-3,
J.
The tests provided results of 4175 psi and 4075 psi respectively.
Based on our observations of the equipment and techniques used we suggest that these result.,be treated as
indicative of the in-situ strength of the material.

Or mast reeent apart. of the Special Inspection program noted that;
The concrete cylinder samples made on June 11 faNd to achieve an ultimate compressive S1:-es capacity
greater than that rcquired by the construction documentation at 2R or Sfi days. However rec,,-nt "Windsor
