for the connection to the Town's sewerage system for commercial property located at 283 Sullivan Avenue;as j

Revision Dates: 3-9-06, 3-16-06; also Drawing No.: Jobs/1650/1650-3.DWG , Sheet No.: 3/4, Dated 1-31-06
with Revision Date: 3-9-06";and subject to the following conditions:
1. An easement must be submitted and approved by the Town Attorney permitting the Town access to the
property for manhole monitoring purposes. i
2. This connection charge will be the minimum commercial charge in effect at the time of connection.

3. Technical approval of the Town Engineering Department. ,
4. Final Authority approval and acceptance of the sanitary sewerage connection will be given once the
sewer line has been installed and inspected by the Town staff,and based upon the written verification of
the Town staff that said sanitary sewer line was installed in substantial conformance with the design j
plans approved by the Authority, and' meeting all Town regulations and appropriate technical
specifications relating to sanitary sewer construction.

This approval letter must be superimposed on the final approval plans.

THE FOLLOWING: Two sets of plans and one copy of the application for the above-
referenced project. Please call me to confirm this application will be on the agenda for the April 4rh
meeting. Planning&Zoning is hearing this project on March 281', however they will not be able to
act on the application until April I I'h. We respectfully request that WPCA review and decide on
this application at the April e meeting, as time is of the essence and the client must go to
