Signatur O r of Property Date V/u! 0z') Sign e of ApplicaKt / Date � �Z�/ ( Z 9 BUCKLAND ROAD BRIDGE SEDIMENT REMOVAL PROPOSED ACTIVITY The proposed activity at the Buckland Road bridge is the installation of temporary sediment barriers along Plum Gully Brook, installation of a temporary stone check dam in the brook immediately downstream of all work, installation of a temporary sedimentation basin for dewatering, removal and disposal of sediments from the bridge, removal and disposal of sediments from about 65' upstream and downstream of the bridge culverts, excavation of embankments on each side of the bridge and on each side of the adjacent Plum Gully Brook, installation of intermediate rip rap mixed with modified riprap on each side of the bridge and on each side of the adjacent Plum Gully Brook; cleaning and removal of the temporary sediment basins and stone check dams, restoration of all disturbed areas to original condition and any and all work necessary to complete the project in accordance with the plans and specifications. The site is located northerly of Deming Street and southerly of Oakland Road, with the following conditions: 1. The final approved copy of the entire set of plans and this letter reproduced thereon must be submitted to the Planning Department. This must be completed within 65 days of approval prior to any construction activity on the site. Plans submitted to Planning & Zoning Commission shall be considered having met this requirement. 2. The application shall indemnify and hold harmless the Town of South Windsor against any liability, which might result from the proposed operation or use. 3. The permit is valid for five years and shall expire on February 15, 2017. It is the landowner(s)/applicant(s) responsibility to track expiration dates and notify the Commission of a renewal request at least 65 days prior to expiration. 4. All approvals required must be obtained and submitted prior to any activity on the site. 5. A contact person shall be identified on the plans. 6. On page one of the plans submitted, under construction notes, wording stipulating "Flow to one box culvert may be temporarily blocked to facilitate cleaning of the other box culvert' should be changed to "Flow to one box culvert should be temporarily blocked to facilitate cleaning of the other box culvert".
