Dear Ms. Mazotas:
The Agency voted to approve your application with conditions at the
September 23 meeting.
Additional erosion and sedimentation controls must be added to your plan,
including hay bales and silt fences. Work out agreeable specifics on
this with the Engineering Department.
The following conditions must also be adhered to for this permit to be valid:
1. Construction activities shall be subject to the approval of the
Town Engineer.
. 2. The application shall indemnify and hold harmless the Town of
South Windsor against any liability which might result from the
proposed operation or use.
3. The permit is valid for one year and shall expire September 23,
1986, if not initiated and completed within one year from the
date of approval.

Dear Ms. Mazotas:
This is a reminder that the permit with the Inland Wetlands Agency/
Conservation Commission, Application # 352 will expire on 9/23/86.
Th4 agency requires a letter requesting extension together with a fee
of $25 . 00 . This should be submitted at least 30 days prior to the
expiration date of the permit. If work has been completed or will be
completed by the expiration date, we would appreciate you contacting
the Inland Wetlands Agency/Conservation Commission' s secretary, Debbie
Bellizzi at 644-2511 Ext. 255 to advise her of work status .
