This provides the parking for all stag, the patient in the chair rcc6vfng treatment, plus the next patient to receive treatment. This calculation assumes that YOU can work in two rooms at one time which typically is not the case, due to the down time of room because of disenfection.procedures. Concluding that out of six rooins you will awy.have three to four patients at one time receiving treatment, another three to four waiting for treatment, plug staff_ Traditionally an office this size would have a max of eight employees, so allowing for sixteen parking spots would be snore than adequate. DATE: July 26, 2005 TO: Peter DeMallie Design Professionals, Inc. 165 South Satellite Road South Windsor, CT 06074 FROM Bruce Hillson -Traffic Engineering Solutions RE: Grillo Dentist Office As requested, Traffic Engineering Solutions, P.C. has completed this memorandum to address following two items that have been raised by Town Staff during their review of the Dentist Office proposed by Dr. Robert M.Grillo for 498 Buckland Road in South Windsor. • The Intersection Sight Distance from the site driveway. • Traffic volumes. Our July 16, 2004 letter report identified the need to remove two trees just north of the site driveway. One of the trees was identified as likely being removed to construct the site driveway and the second was identified as likely being in front of the abutting property. We have reviewed the latest plans for the proposed Dentist Office and found that one of the trees will in fact be removed during construction of the driveway. A recent field visit by a representative from Design Professionals, Inc. indicates that the second tree has been removed during the past year and only a low tying stump remains. The existing obstructions to the sight distance looking to the north have either been removed or will be removed when the new driveway is constructed. The traffic volumes used for our analyses were taken from the traffic studies completed for the Evergreen Walk and Stop&Shop developments. The traffic volumes that were used were for the 2004 Build conditions for these other two developments. For the analyses we completed for Dr. Grillo's dentist office,we increased the traffic volumes from the Evergreen Walk and Stop&Shop developments by two percent to account for any other background growth that may occur in the area. We would not expect any significant change in operating Levels of Service from those presented in our July 16, 2004 letter, If during review of this memo you have any questions, please call me. Bruce CABUSINESS\ GRILLOSIGHTLINEWEN10072605-DOC i Traffic Engineering Setafient P.C. 193 Lexington Road Glastonbury,CT 06033 (860)657-3579 July 16,2004 Robert M. Grillo,DMD 47 Burnside Avenue East Hartford, CT 06108 Dear Dr. Grillo: Traffic Engineering Solutions has completed a study to determine the operating Level of Service and adequacy of intersection sight distances at the proposed access drive to your proposed 3,158 square foot dentist office at 498 Buckland Road in South Windsor. The following work was included in this study: • Reviewing traffic impact studies prepared by others for the Stop &Shop supermarket and Evergreen Walk developments on Buckland Road in South Windsor to determine the morning and afternoon peak hour traffic volumes for Buckland Road. • Making 24-hour directional machine counts on Buckland Road in the vicinity of the site. The count included a directional speed study to determine the 85th percentile speed of travel in each direction on Buckland Road(85th percentile speed was used to determine the intersection and stopping sight distances for vehicles entering and leaving the site). • Coordinating with Design Professionals,Inc.to have their survey crew obtain Intersection Sight Distances when looking left and right from the proposed site driveway. • Making visual observations of the roadway and land use conditions along Buckland Road in the vicinity of the site. • Determining the number of trips expected to be added to the roadway system by the proposed dentist office. • Using the traffic count information from other traffic studies, completing morning and afternoon peak period capacity analyses for the intersection of Buckland Road at the site access drive to determine the operating Levels of Service(LOS)for the future traffic volumes anticipated upon completion of the dentist office. • Comparing the intersection sight distances measured by Design Professionals, Inc. with the Connecticut DOT Guidelines for Intersection Sight Distances for the 85th percentile speeds obtained from other studies. cAbusiness\Gri11osi0TtLine\1trrptdoc Existing Conditions Robert M. Grillo,DMD proposes to rehabilitate and expand the existing single family home located at 498 Buckland Road to create a new 3,158 square foot dentist office for his use. The site is situated along the east side of Buckland road approximately 700 feet south of Ellington Road. Buckland Road in the vicinity of the site is 52 feet wide with two travel lanes in each direction. A double yellow center line separates the two directions of travel and dashed white lines define the two lanes of travel in each direction. Solid white edge lines delineate shoulders along both sides of the road. The posted speed limit on Buckland Road in the vicinity of the site is 40 miles per hour. Buckland Road is straight and flat in the immediate vicinity of the site. South of the site the road goes up a slight hill. A directional machine count and speed study was made on Buckland Road along the site frontage from June 26 through July 6,2004. The traffic count indicates that the daily traffic volume on Buckland Road varies from 15,000 to 15,500 vehicles. The traffic studies prepared for the Evergreen Walk' and the Super Stop&Shop at Town Center2 were used to identify the morning and afternoon peak hour traffic volumes on Buckland Street along the site frontage. The Evergreen Walk study indicates that there would be 1,276 vehicles traveling Buckland Road during the morning peak hour(357 vehicles traveling northbound and 919 vehicles traveling southbound). The Stop&Shop study indicates that there would be 2,031 vehicles traveling Buckland Street during the afternoon peak hour(1,220 vehicles traveling northbound and 811 vehicles traveling southbound). The machine count included a sped study which indicated that the 85t'percentile speed of travel on Buckland Road is 44 miles per hour for both northbound and southbound travel. Trip Generation New trips to the roadway system generated by the proposed 3,158 dentist office were determined from the Institute of Transportation Engineers (ITE)reference, Trip Generation . The ITE reference has established mathematical relationships based on studies of various land uses to determine their trip generation rates. These trip generation relationships have been standardized and published in the Trip Generation reference. The ITE reference provides trip generation information for several related land uses for senior housing including: "Medical-Dental Office Building". The trip generation was based on Land Use Code 720-"Medical-Dental Office Building". The following trip generation relationships were used to determine the number of daily, and morning and afternoon peak hour trips that would be generated b the proposed 3 158 square foot dentist office. The results are p g Y P p q summarized in Table 1. Medical-Dental Office Building Average Weekday Trips 36.13 Trips per 1,000 square feet Morning Peak Hour 2.48 Trips per 000 square feet 79/21 Afternoon Peak Hour Ln(T)=0.93 Ln(X)+ 1.47 27n3 where T is the number of trips and X the building size in 1,000 square feet 'Traffic Impact Report prepared by F.A.Hesketh&Associates,Inc.April 18,2001 2 February 22,2002 traffic analysis submitted to the State Traffic Commission by F.A.Hesketh&Associates,Inc. a Trip Generation 7th Edition published by the Institute of Transportation Engineers,2003 cAbusiness\GrilloSightLineUtript.doc 2 Table 1 - Trip Generation- Senior Housing Trips Entering Trips Leaving Daily 57 57 Morning Peak Hour 6 2 Afternoon Peak Hour 3 9 Table 1 indicates that the proposed 3,158 square foot dentist office will generate new 8 trips during the morning peak hour with 6 vehicles entering the site and 2 vehicles leaving the site. During the afternoon peak hour the dentist office will generate 12 new trips with 3 vehicles entering the site and 9 leaving. The New Trips determined from the equations and shown in Table 1 were used with the analyses completed for this study Capacity Analyses Capacity Analyses were done to determine the anticipated quality of traffic operations at the intersection Buckland Road and the site access drive. Letter designations from A to F are used to represent the Levels of Service(LOS)for the traffic operation at each intersection or roadway, with LOS A representing the best operating conditions and LOS F the worst. The Level of Service for unsignalized intersections with STOP control on the minor street assumes that through and right-turning traffic on the main street is not affected by traffic on the side street(s). Thus, the LOS designation is for the movements on the side street and the left-turn movement from the main street onto the side street. Levels of Service are defined by the average delay per vehicle as indicated below. Unsignalized Intersections Level of Service Avg. DelayNehicle(in Seconds) LOS A < 10.0 LOSB > 10.0 and< 15.0 LOS C > 15.0 and<25.0 LOS D >25.0 and<35.0 LOS E > 35.0 and<50.0 LOS F >50.0 Level-of-service capacity analyses were completed for the 2005 Build conditions for the intersection of Buckland Road at the site access drive. The year 2005 analyses assume a one year buildout to renovate and expand the existing structure to a 3,158 square foot dentist office. The 2005 traffic volumes on Buckland Road were determined by applying a two percent growth factor to the background traffic volumes on Buckland Road to account for growth that may occur in the area. Trips generated by the dentist office were added to the 2005 traffic volumes on Buckland Road assuming that two-thirds of the new trips would travel to and from the south and two-thirds would travel to and from the north on Buckland Road. This approach provides a conservative analysis of the impact from the site trips as the total number of trips assigned to the roadway system represents 133%of the generated trips. The results of the capacity analyses are presented in Table 2. cAbusiness\GriltoSightLine\itnptdoc 3 Table 2: Unsignalized Intersection Capacity Analysis 2005 Build ,r,o . Table 2 shows that left turns from Buckland Road onto the site access drive will operate at Level of Service(LOS)A during the morning peak hour and LOS B during the afternoon peak hour upon completion of the proposed dentist office. The site drive approach to Buckland Road will operate at LOS B during the morning peak hour and LOS E(41 seconds average delay per vehicle)during the afternoon peak hour. The capacity analyses also provide stacking information for left turns from Buckland Road onto the site drive and on the site drive approach to Buckland Street. The 95`b percentile queue(there is a 95 percent statistical probability that this queue will not be exceeded)for left turns from Buckland road onto the site drive will be virtually non existent during both peak periods and the queue on the site drive will be virtually non existent during the morning peak hour and less than half a vehicle during the afternoon peak hour. Site Access Access to the dentist office will be provided by an access drive located along the east side of Buckland Road at the northerly property line of the site(approximately 700 feet south of Ellington Road. Visual observations were made at the location of the site access drive, and Design Professionals,Inc. (DPI)made intersection sight distance measurements looking left and right from the access drive. DPI measured the Intersection Sight Distances as being at least 700 feet looking north and south from the site drive. The visual observations indicate that the 700 foot Intersection Sight Distances can be achieved in accordance with the 2003 ConnDOT Highway Design Manual (measured at a distance of 15 feet off the edge of road with a 3.5 foot height of eye and height of object)by removing two trees just north of the site driveway(one will likely be removed to allow construction of the drive and the second tree is near the property line and may be on the abutting property)and removing trees and shrubs along the site frontage south of the site drive. The Connecticut DOT presents intersection sight distances(ISD)in their December 2003 Highway Design Manual. Figure 11-2D of the ConnDOT Guidelines presents ISD information for each 5 mph increment for speeds between 20 and 70 mph for four-lane roads. The posted speed limit on Buckland Road in the vicinity of the site is 40 miles per hour(mph). The speed study conducted for Buckland Road at the site frontage found that the 85t'percentile speed(the speed at which 85 percent of all drivers travel at or less than)is 44 mph for both northbound and southbound travel. A summary of available intersection sight distances and distances required by ConnDOT in their Guidelines for Highway Design is presented in Table 3. Obusiness\CmlloSightLine\ltapt.doc 4 Table 3—Comparison of Available and Required Intersection Sight Distances Available Intersection Required Intersection Sight Sight Distance Distance per ConnDOT 2003 Highway Design Manual Looking Left from Site Drive onto >700 Feet 519 Feet Buckland Road Looking Right from Site Drive 700 Feet 519 Feet onto Buckland Road Table 3 shows that the Intersection Sight Distance(ISD)available to drivers looking left and right from the site access drive onto Buckland Road(with the recommended clearing)is greater than the intersection sight distance provided in the ConnDOT reference for vehicles traveling at the 85'h percentile speed of 44 mph in the northbound and souhbound directions on Buckland Road. A second sight distance measured for this study is for Stopped Vehicle Turning Left Across Oncoming Traffic. According to Figure 11-2J of the ConnDOT Guidelines, vehicles turning left from Buckland Road onto the site drive should be able to see 391 feet. The 700+feet available to drivers making this left turn is greater than the distance required by the ConnDOT reference for vehicles approaching from the opposite direction at the 851'percentile speed of 44 mph. The Stopping Sight Distance for vehicles approaching from the rear of the vehicle turning left onto the site access drive should be approximately 349 feet. The available Stopping Sight Distance(700 feet)is greater than the required Stopping Sight Distance. All sight distances available to drivers entering and leaving dentist office drive(with the recommended clearing)are greater than the distances presented in the Connecticut DOT 2003 Highway Design Manual for the prevailing travel speeds on Buckland Road. These sight distances will allow drivers the visibility needed to safely enter and leave the site. Conclusions Robert M. Grillo, DMD proposes to rehabilitate and expand the existing single family home located at 498 Buckland Road to create a new 3,158 square foot dentist office for his use. The site is situated along the east side of Buckland road approximately 700 feet south of Ellington Road. The trip generation information included in the ITE Trip Generation reference for Medical- Dental Office Building was used to determine the number of new trips anticipated from the proposed 3,158 square foot dentist office. The study assumed that the dentist office would be constructed over a period of one year. Capacity analyses were done for the intersection of Buckland Road at its intersection with the site access drive for the 2005 Build conditions. The trips generated by the dentist office were distributed to the roadway system by assuming that two- thirds of the new trips would travel to and from the south and two-thirds would travel to and from the north on Buckland Road. This approach provides a conservative analysis of the impact from the site trips as the total number of trips assigned to the roadway system represents 133%of the generated trips. The capacity analyses found that left turns from Buckland Road onto the site access drive .N411 operate at Level of Service(LOS)A during the morning peak hour and LOS B during the afternoon peak hour upon completion of the proposed dentist office. The site drive approach to cAbusinessl GrilloSightLinAtrrpt.doc 5 Buckland Road will operate at LOS B during the morning peak hour and LOS E(41 seconds average delay per vehicle)during the afternoon peak hour. The capacity analyses also provide stacking information for left turns from Buckland Road onto the site drive and on the site drive approach to Buckland Street. The 95th percentile queue(there is a 95 percent statistical probability that this queue will not be exceeded)for left turns from Buckland road onto the site drive will be virtually non existent during both peak periods and the queue on the site drive will be virtually non existent during the morning peak hour and less than half a vehicle during the afternoon peak hour. Design Professionals, Inc. (DPI)made intersection sight distance measurements looking left and right from the access drive and visual observations by Traffic Engineering Solutions,P.C. indicate that the 700 foot Intersection Sight Distances measured by DPI can be achieved in accordance with the 2003 ConnDOT Highway Design Manual(measured at a distance of 15 feet off the edge of road with a 3.5 foot height of eye and height of object)by removing two trees just north of the site driveway(one will likely be removed to allow construction of the drive and the second tree is near the property line and may be on the abutting property)and removing trees and shrubs along the site frontage south of the site drive. Sight distances for turning left from Buckland Road onto the site and stopping sight distance for vehicles following these left turning vehicles exceed the requirements presented in the ConnDOT Highway Design Manual. Traffic operations at the intersection of Buckland Road at the site access road will be very good Levels of Service A or B for left turns from Buckland Road onto the site access drive and the site drive approach to Buckland Road will operate at LOS B during the morning peak hour and LOS E during the afternoon peak hour with queues of less than half a vehicle during the afternoon peak hour. Adequate sight distances can be provided at the site access drive by clearing trees and vegetation to provide drivers the visibility needed to safely enter and leave the site. No offsite roadway improvements are suggested by this study as a result of the added traffic generated by the proposed dentist office. Grillo The Grillo Family-Limited Partnership 379 Main Street South Windsor, CT 06074 Dear Dr. Grillo: Re: Appl#05-38P, Grillo Site Plan We regret to inform you that the Planning and Zoning Commission, at their August 16, 2005 meeting, voted to deny without prejudice, the above referenced application for site plan approval. The reason for the denial was that the site plan did not appear to conform to the condition set by the Zoning Board of Appeals in granting the necessary variances on the property that the plans be substantially similar to the plans submitted to the ZBA. The denial was without prejudice to allow you to resubmit your site plan application either after it to conforms to the condition imposed by the ZBA or after you have received clarification from the ZBA that the current site plan conforms to that condition. The Board concluded the hardships and conditions on each request to be as follows: A 13' variance to section 10.2 to allow a side yard of 12' (25' required): Hardship: 1) The placement of the original structure conformed at the time it was built and the re- zoning of the property to the Buckland gateway zone requires larger setbacks. 2) The property line is not perpendicular to the structure, making the expansion encroach further into the side yard. 3) The two government takings for road widening have created the existing structure to become significantly nonconforming, making it difficult to use the property in conformance with the Town's comprehensive plan. Conditions: 1) Approval is based on the updated plans prepared by Design Professionals, Inc., last revision date of 7/1/2005. 2) The use is subject to site plan approval by the Planning &Zoning Commission. 3) The use is subject to an approved use in the zoning regulations. MEMORANDUM TO: Marcia Banach, Director of Planning FROM: Jeff Doolittle, P.E., Town Engineer Joseph J. Perna, Project Engineer DATE: January 12, 2006 RE: Appl. No. 05-73P - Grillo Site Plan We have reviewed the site plan for the Grillo Family Limited Partnership located at 498 Buckland Road and offer the following comments: 1. Plans call for a new sanitary sewer connection. Our records indicate that house #498 was connected to the sanitary sewer on 4/12/79. This sanitary sewer lateral needs to be shown and should continue to be used if possible. If it can not be used it needs to be disconnected and capped at the street line. Note 9 regarding the existing septic system on sheet 2A needs to be changed. 2. Provide inverts for the proposed foundation drain connecting to the existing catch basin on Buckland Road. 3. The sidewalk extension on the north side of the proposed driveway should be constructed as a handicap ramp. 4. WPCA review and approval is needed for this application Please let us know if you have any questions. Appl #05-73P, Grillo Family Limited Partnership January 24, 2006 Planning Department Report l. Request for site plan approval to convert an existing residence into a dental office at 498 Buckland Road, Gateway zone. This applicant was in front of the Commission in July of 2005 and the application was denied because the site plan did not appear to conform to the condition set by the Zoning Board of Appeals in granting the necessary variances on the property that the plans be substantially similar to the plans submitted to the ZBA. The applicant has subsequently reapplied to the ZBA and was granted the necessary variances to move forward with this application in December 2005. A copy of the ZBA letter has been provided. 2. Several variances have been granted on this property. In April 2004, ZBA granted variances to allow a 12' side yard rather than the 25' required in the gateway zone. In December 2005; the ZBA and to allow a 37.9 front yard setback for all structures on the property (65' required). 3. Maximum impervious coverage allowed is 60%, 35% proposed. Proposed building height is 18 feet; 30 feet allowed. Lot size is 48,300 sq ft; minimum lot size allowed is 3 acres. The Gateway zone specifically acknowledges existing undersized lots and notes that they are legally nonconforming. Frontage is 184 feet; minimum allowed is 200 feet. 4. This application is subject to the requirements of Section 3A, access management, as well as the access management requirements of Gateway Zone section 5.8.2.e, which states: "Access management will be required on all sites, in order to reduce the number of driveway cuts onto Buckland Road and mitigate the deterioration of traffic flow generally caused by driveways on arterial streets. Access management techniques will include shared driveways (or provisions for future shared driveways for the first site in an area); interior service drives; and other techniques included in South Windsor's access management program." It is critical that provisions be made for interconnections to both the north and south properties so that in the future, these properties can be interconnected and share a single driveway. The site plan reflects this provision by showing an area of future connection to the north and south. If this application is approved, there needs to be an approval condition requiring that this site be interconnected with the two abutting properties when the properties receive site plan approval in the future. i A The Police Department strongly recommends that a shared driveway be utilized to serve the subject site and abutting sites. Sgt. Bond also reminds us of the left- turn prohibition at CVS and suggests that full access be granted only if a common driveway is established. 5. A sidewalk has been shown along the Buckland Road frontage as required by the Gateway Zone regulations. Staff note that sidewalks are being introduced along Buckland Road at each opportunity; e.g., a sidewalk for Berry Patch senior housing is coming down to Buckland Road and will travel along Buckland Road frontage to cross at the Stop & Shop/Terry Office Park traffic light. 6. The parking requirement for this use is 26 spaces. The applicant is requesting a waiver to allow 22 spaces. The Commission will want to consider whether you agree that 22 spaces is adequate now and in the future, since there is no room for expansion of the parking lot. We do note that future interconnection to the sites north and south could provide possibilities for shared parking in the future. 7. Architectural and Design Review Committee reviewed this application on June 16. They were satisfied with the architecture and landscaping. ADRC was concerned that a sidewalk is being required because there are no sidewalks to the immediate north or south at present, and ADRC thought the sidewalk was too close to Buckland Road for safety. 8. Proposed lighting includes decorative, full cutoff fixtures on 14-foot poles. 9. There is a retaining wall along the south boundary that will need to be reconfigured in the future when the interconnection to the south is constructed. 10.The proposed sign is a very tasteful 4' high illuminated monument sign. We do remind the applicant that when lighting the monument sign, care will be needed to ensure that the lighting conforms with zoning requirements. 11.There are regulated wetlands on the site. IWA/CC approved the application on July 6 with no unusual approval conditions and two $5,000 bonds for erosion and sediment control, and for detention basin establishment. 12.Public water and sewer are available. Water Pollution Control Authority approval is required. 13.There is a dumpster shown on a concrete pad with screening. 14.If this application is approved, staff request an approval modification requiring the driveway interconnections at such time as site plans are approved and construction occurs on the abutting sites. 2 sow ?d 1540 SULLIVAN AVENUE • SOUTH WINDSOR, CT 06074 sox co`'` TELEPHONE (860) 644-2511 CERTIFIED MAIL#70041160 0007 4128 2294 February 6, 2006 The Grillo Family Limited Partnership 379 Main Street South Windsor, CT 06074 Dear Dr. Robert M.
