Table of Contents: Page# Introduction I Proposed Development I Off-Site Analysis I Erosion/Sedimentation Control Systems I Conveyance System I Discharge at the Natural Location I Maintenance and Operation 1 Topographic Contour Maps 2 Floodplain Boundaries 2 On-Site Storm Sewer Calculations 2 Conclusions 2 Appendices Storm Sewer Calculations A Introduction: The site is located on the west side of Buckland Road in South Windsor, Connecticut, and consists of a single parcel of land. There is presently a converted house on the site that contains the existing hair salon. The site is boardered on three sides by the recently approved "The Shops Of Evergreen Walk" development. There are existing wetlands to the north and south of the site, with a very small amount of wetlands on the southeast corner of the site. Proposed Development: The proposed development of the site includes the construction of a 2-story, 9,000 S.F. office building that will also house the Vision's Salon and Spa. As part of the development, parking, and driveways will be constructed. The site will be serviced by public sanitary sewer and water. The Storm drainage system proposed for the development includes the installation of storm-sewers within the proposed parking areas that will connect into and discharge through The shops of Evergreen Walk. The runoff from the parking areas along the eastern side of the building will discharge to a low vegetated area along the eastern side of the site where infiltration will occur. In the event that this area fills with water, the overflow will enter the storm drainage system via 2 catch basins in this area. Off-Site Analysis: The site through an easement and agreement with The Shops of Evergreen Walk will discharge into the storm system constructed as part of this development. Erosion/Sedimentation Control Systems: An extensive erosion/sedimentation control plan has been developed as part of the proposed site design. These measures include the following: 1. Installation of silt fence down-slope of disturbed areas. 2. Installation of silt fence around stockpiled materials. 3. Installation of silt fence around catch basins. 4. Seeding and mulching disturbed areas as soon as practical during construction. 5. Limiting the areas of disturbance during construction. 6. Installation of construction entrance pads, and sweeping of roadways as needed. 7. Maintenance of the above measures. Conveyance System: The on-site conveyance system consists of a piped stormwater system connected into The Shops of Evergreen Walk. The piped drainage system has been designed to accommodate the runoff from a 10 Yr design storm. Discharge at the Natural Location: The site presently drains to the west, and will continue to do so under developed conditions. Maintenance and Operation: The following maintenance operations will be required to ensure the proper and efficient operation of the Storm Water System. The following maintenance schedule is intended to be a minimum guide, and additional inspections and maintenance measures may be required following large storm events that could cause the deposition of excess debris in the system. Catch Basins: The catch Basins shall be inspected annually. The sumps shall be cleaned when the depth of material within the sumps reaches 1'. Pavement Sweeping: The parking lots shall be swept at a minimum of twice per year. Once in the spring shortly after the end of the snow season, and once in the fall after the leaves have fallen. Topographic Contour Maps: A topographic contour map has been provided showing the drainage areas for the site at a scale of F'= 20'. The map showing the developed conditions is included in the rear of this report. Floodplain Boundaries: There are no floodplain boundaries on the site. On-Site Storm Sewer Calculations: The storm sewer calculations for the storm piping within the proposed parking areas are included in appendix A. The storm sewer has been designed for a 10-year design storm utilizing the rational formula. Conclusions: The on-site stormwater system will adequately handle the runoff from the site development. RE: Vision's Salon & Spa 207 Buckland Road South Windsor, Connecticut STC Certificate No. 1533 (Evergreen Walk) Our File: 00167.00 ' Dear Ms. Cabelus: On behalf of Vision's Salon & Spa Corporation, we are herewith submitting (9)copies of an Application-for Certificate to modify the above referenced Certificate to include a proposed development known as Vision's Salon & Spa located at 207 Buckland Road in South Windsor, Connecticut, The.proposed development is to consist of a single 9,000 s.f. building to be occupied by Vision's Salon & Spa. A totsI of 43 parking spaces will be provided on site, including 2 handicapped spaces. Access to the site will be provided through the Evergreen Walk Development, there will be no direct access to local roadways_ We are therefore requesting a modification to the Evergreen Walk Certificate pursuant to Section 14-311 (c) of the Connecticut General Statutes, as amended. Evergreen Walk is a mixed-use development approved under the State Traffic Commission Certificate No. 1533 for a total of 1,230,000 s.f. and 5,200 parking spaces. Evergreen Walk is expected to generate 1,341trips during the morning peak hour, 2,024trips during the afternoon peak hour, and 1,924 trips during the Saturday peak hour. The approved site plans provide a total of three driveways to Buckland Road and .one driveway to Deming Road. The Institute of Traffic Engineer's (ITE) publication Trip Generation was utilized to compute the anticipated volume of traffic to be generated by the proposed development, Since there was'no salon facility use listed in ITE,trip generation computations were performed for several land uses that could expect to see a similar generation rate. The most conservative results were for an athletic club, with 28 vehicle trips occurring during Civil &Traffic Engineers• surveyors• Planners- Landscape Architects Tel 960.653.8000• Fax 860.844.8600• email maikgfahesketh.com 05/18/2004 16:18 8608448600 FAHESKETH PAGE 02 Ms. Robbin Cabelus May 18,2004 Page 2 the morning peak hour, 52 during the afternoon peak hour, and 33 trips during the Saturday peak hour. The proposed increase in traffic on the Evergreen site driveways as a result of the proposed Spa is therefore minimal. The trip generation computations are attached. We therefore ask that the Evergreen Walk Certificate No. 1533 be modified to a total of 1,239,000 s.f. and 5,243 parking spaces. We trust that the information provided is sufficient for your office to undertake a review to amend the certificate. Application No. 04-37P—Vision's Salon& Spa Vision's Salon& Spa Corporation West Side of Buckland Road(approx. 1,280 feet south of Deming Street) Gateway Development Zone South Windsor, Connecticut VERIFIED PETITION TO INTERVENE Pursuant to Connecticut General Statutes ("CGS") Section 22a-19, Pavilions at Buckland Hills L.L.C. ("Pavilions at Buckland Hills")hereby intervenes in this administrative proceeding by the filing of this Verified Petition to Intervene and requests that it be granted status as an intervening party in this administrative proceeding before the South Windsor Planning and Zoning Commission("P&Z Commission") concerning the proposed development of a an office building and accompanying improvements to house at least Vision's Salon& Spa. In support of its request,Pavilions at Buckland Hills represents, upon information and belief, the following: 1. Vision's Salon& Spa Corporation("Vision's") proposes to construct a 9,000-square- foot, two-story office building, parking areas, driveways, stormwater structures, and associated improvements known as Vision's Salon& Spa on 40,324 square feet(0.96 acres) located on the west side of Buckland Road, approximately 1,280 feet south of Deming Street. As part of the process to obtain the necessary approvals to construct the proposal, Vision's has submitted an application to the P&Z Commission for approval of the proposed development. The application states or shows the following: a. The proposal includes the construction of parking areas, driveways, stormwater structures, and associated improvements. b. The proposal appears to require the razing of an existing, formerly residential structure on site. c. The plans show substantial alteration and filling of the site. d. Wetlands comprise approximately 2,823 square feet(0.065 acres) of the proposed site. The proposed development will require the disturbance of 27,818 square feet (0.63 acres) of the regulated wetlands buffer area on the site, including the clearing of wooded areas, the placement of stone, gravel, and topsoil as fill, and the construction of extensive areas of impervious surface. 2. The P&Z Commission is authorized to grant, grant with conditions, or deny permits or site plans for all proposed uses of land and activities thereon within South Windsor. 3. In its review of the application of Vision's, the P&Z Commission will consider"conduct which has, or which is reasonably likely to have, the effect of unreasonably polluting, impairing or destroying the public trust in the air, water or other natural resources of the 367217 state." CGS Section 22a-19. The construction and operation of Vision's Salon& Spa involves, inter alia, such conduct in one or more of the following ways: a. Loss of open space within South Windsor, contrary to the Open Space Master Plan and the draft Plan of Conservation and Development; b. Erosion of soils and stormwater runoff during construction; c. Increase of stormwater flow from the construction of impervious surface for parking areas, driveways, and walkways, which will increase sedimentation and degrade surface water and groundwater quality; d. Exacerbation of flooding by greatly increasing the extent of impervious surface and by inhibiting the function of area wetlands; e. Harassment, displacement, and possible wounding and killing of wildlife; f. Destruction of wildlife habitat; g. Increase of traffic congestion along Buckland Road and Deming Street, and the possible degradation of local air quality; and h. Future undefined but inevitable short- and long-term impacts given that this proposal is part of and/or adjacent to a larger proposed development. 4. Pavilions at Buckland Hills, in accordance with its rights pursuant to CGS Section 22a- 19, respectfully requests all rights of participation granted to any party within administrative proceedings before the P&Z Commission. The request of Pavilions at Buckland Hills for party status will further the policies and purposes of the Connecticut Environmental Protection Act, CGS Sections 22a-14 through 22a-20. 5. The proposed development of Vision's Salon& Spa encompasses myriad environmental impacts. The participation of Pavilions at Buckland Hills within the proceedings before the P&Z Commission will advance the public interest in the protection of the natural resources of the state. 6. In accordance with CGS Section 1-227,Pavilions at Buckland Hills requests that it be provided with written notice by mail of all regular and special meetings of the P&Z Commission concerning Application No. 04-37P and/or Vision's Salon& Spa at least one week before the date of each such meeting, as well as all negotiations pertaining to the above captioned application. 367217 2 WHEREFORE, Pavilions at Buckland Hills intervenes in this action on the filing of this Verified Petition to Intervene, requests notice of all meetings, and further requests that its Verified Petition to Intervene be granted. In the Matter of: July 2, 20 Application No. 04-37P—Vision's Salon& Spa Vision's Salon& Spa Corporation West Side of Buckland Road(approx. 1,280 feet south of Deming Street) Gateway Development Zone South Windsor, Connecticut VERIFIED PETITION TO INTERVENE Pursuant to Connecticut General Statutes("CGS") Section 22a-19, Pavilions at Buckland Hills L.L.C. In support of its request, Pavilions at Buckland Hills represents, upon information and belief, the following: 1. 3. Loss of open space within South Windsor, contrary to the Open Space Master .Plan and the draft Plan of Conservation and Development; b. 4. Pavilions at Buckland Hills, in accordance with its rights pursuant to CGS Section 22a- 19,respectfully requests all rights of participation granted to any party within administrative proceedings before the P&Z Commission. The request of Pavilions at Buckland Hills for party status will further the policies and purposes of the Connecticut Environmental Protection Act, CGS Sections 22a-14 through 22a-20. 5. The proposed development of Vision's Salon&Spa encompasses myriad environmental impacts. 367217 2 WHEREFORE,Pavilions at Buckland Hills intervenes in this action on the filing of this Verified Petition to Intervene, requests notice of all meetings, and further requests that its Verified Petition to Intervene be granted. Box 231277 Hartford, CT 06123-1277 Re: Petitions to Intervene in Applications for L. A. Fitness and Visions Salon and Spa before the Planning and Zoning Commission Dear Atty. King: The Petitions to Intervene which you filed with respect to the above-captioned applications are expected to be considered and acted upon by the South Windsor Planning and Zoning Commission at its meeting of August 10, 2004. Virtually identical petitions were filed in the same applications before the Inland Wetlands Agency and were granted upon the condition that the individual signing the Verification provide specific information of the identity of the consultants to whom he refers in the Verification, upon which he claims, at least in part, to base his opinion and the facts alleged in his Verification. As I indicated during the proceedings before the Inland Wetlands Agency, at which you were present, it is my opinion that this information is necessary for the applicant to establish the basis for his personal knowledge of the claims made in the Petition and the Verification. Without this information, the Petitions are, in my opinion, deficient, and I will recommend that they not be granted without it. This letter is simply to advise you that my recommendation to the Planning and Zoning Commission will be the same as the one to the Inland Wetlands Agency. John C. King, Esq. Page 2 August 5, 2004 It is possible that the Public Hearing will be closed on the evening of August 10, 2004, so you should be prepared with information regarding the Verification that evening and also be prepared to go forward with your evidence. This letter is simply to provide advance notice to you so that you may be properly prepared. The precise course that the Planning and Zoning Commission chooses to take is, of course, up to them and they need not follow my recommendations. In the event that they are inclined to follow my recommendations, however, I wanted you to be aware of the information which would be required for you to proceed. If you have any questions regarding the above, please do not hesitate to contact me. Very truly yours, B . Doolittle, P.E., Town Engineer Date: 8/9/04 RE: App 04-37P, Visions Salon and Spa Site Plan I have reviewed the revised site plan (dated 7/30/04) for the Visions Salon and Spa at 207 Buckland Road and I have the following comments: 1. There is a note on the plans about a new 2" water tap and service by water company. This should be installed prior to the completion of the widening and median construction on Buckland Road to minimize disturbance of the road. 2. The existing water service should be abandoned off the pavement so the new pavement is not disturbed. 3. These plans do not agree with the Buckland Road widening plans. They show an 8 foot wide median island and the water main about 5 feet to the east of this median. Check this and correct the Visions plans as necessary. 4. Check the invert elevation shown for CB 2. 5. The Layout Plan includes a note that proposed sidewalk along Buckland Road to be by Poag & McEwen. The sidewalk in front of this property needs to be completed before a certificate of occupancy can be issued for the new building. 6. The perforated pipe by the infiltration basin is 6" on the plan but 4" on the details on sheet 12. Correct the pipe size and location on the details. 7. The drainage report needs to be revised to show the pre and post development runoff from the 2, 10, 25, 50, and 100 year storms. There needs to be detention and water quality treatment measures for the stormwater from this property. If the stormwater is discharging through an adjacent developed site it must be shown that the stormwater facilities are sized to handle and treat stormwater from this property. 8. Street Excavation Permits must be taken out for each lot by the contractor before the installation of the driveways and water services. 9. An application must be made to the WPCA for the Sanitary Sewer connections for this site. Please let me know if you have any questions. All erosion and sedimentation control measures are to be in place prior to earth moving_operations in each area and maintained/cleaned out during construction. Call Jeffrey Folger, Environmental Planner for inspection when measures are in place, prior to commencement of site work or if any problems/concerns arise during construction. 2. All topsoil stockpile areas to be protected(ringed with haybales or equal). 3. During construction of the road/driveway a crushed stone tracking pad is to be installed and maintained to minimize tracking of dirt/gravel on to Buckland Road. Calcium chloride or water is to be used for dust control as required. Public Works 1. Three complete sets of approved/revised plans to be provided to Town prior to start of construction. They will be stamped by the Planning Department along with plans developer needs. Only approved plans are to be used on the job site. 2. The Engineering Department is to be notified forty-eight (48) hours prior to the start of major construction work with inspections and testing per Developers Checklist for the following: a. After site is staked out y. Sanitary sewer connections All drainage work If a proposed driveway location is moved, the sidewalk must be removed to the nearest expansion joint and rebuilt to the proper depth with reinforcement. 18. During construction of individual sanitary sewer house laterals, the contractor shall prepare an as-built sketch on standard form provided by the Engineering Department. This form should be submitted to the design engineer/surveyor for their use in preparing the as-built plan. The Engineering Department suggests that the contractor also show locations of foundation drains and water laterals on this form. 19. During construction for individual house lots, lot to lot drainage must be addressed with drainage swales as necessary. For lawn establishment, town staff will be requiring erosion control measures to be installed prior to the issue of the certificate of occupancy to ensure soil stabilization. A detailed erosion control plan must be implemented and depicted on the plot plan for review prior to the issuance of the building permit—most particularly on lots with extensive grading. Planning Department 1. Landscaping is to be installed in accordance with the approved landscaping plan. If there are changes required, contact the Planning Dept. Additional landscaping installed on one part of a site will not preclude planting all of the remaining items shown on the landscaping plan. 2. All bonds must be posted and the mylar must be signed and filed prior to issuance of building permits. 2. A sign permit is required for all building and free standing signage. 3. All easements must be put on file in the office of the Town Clerk. SNET/NU 1. Contact utilities for coordination of utilities connection. Fire Marshal 1. Blasting(if required)to be coordinated with the Fire Marshal. Police Services 1. Emergency phone numbers for the duration of the construction must be provided to Police Services on a company letterhead. If the developer occupies or owns the building after construction emergency numbers must be provided to Police Services. 2. All buildings located on property that abuts a public highway must be numbered and visible from the street. All numbering shall comply with Town Ordinance 6-800 through 6-802. 3. Barns, garages, and outbuildings, which are located on the same premises as a numbered structure, are excluded. 4. Erect temporary sign on the site during construction identifying the project. If an alarm is to be installed contact Police Services.
