At the October 2, 1996 regular meeting, the Inland Wetlands Agency/Conservation Commission
approved application #96-65W, Paul Oates - IWA/CC permit for the construction of a barn on property
located at 44 West Road, RR zone (Assessor's map #87, parcel 12) as shown on plans submitted by
Palmberg & Russo and dated February 16, 1989. This approval is subject to the following conditions:
1. The application shall indemnify and hold harmless the Town of South Windsor against any liability
which might result from the proposed operation or use.
2. The permit is valid for five years and shall expire on October 2, 2001. It has been IWA/CC's
policy to notify landowner(s)/applicant(s) of expiring wetland permits. However, it is the
landowner(s)/applicant(s) responsibility to track expiration dates.and notify the Commission of a
renewal request at least 65 days prior to expiration.
3. All approvals required must be obtained and submitted prior to any activity on the site.

TO THE APPLICANT:
Section 22a-39-14 of the Regulations of Connecticut State Agencies requires standardized reporting of
local wetland agency decisions to the Commission of the Department of Environmental Protection. In
keeping with this requirement the Inland Wetlands Agency/Conservation Commission must submit the
form to the State after a decision is made.
Therefore, at the time the application is submitted, the application form must be filled out excluding
items 9, 10, 11, and 12. You are also res nsible for the photocopy of the USGS Quad map location
(scale 1" to 20001 as per the instructions item #2) on the back of the form. The USGS (Manchester
Quad) map is available at the Planning Department and photocopies can be made in the Town Clerk's
office.
