Project title should be shown on all sheets of the drainage calculations . 2 . Calculations indicate a total flow of 115 cfs but do not show how this was calculated. 3 . Drainage calculations should have signature and stamp of a Professional Engineer licensed in the State of Connecticut. 4 . A cross section, taken perpendicular to the pipe, showing proposed grading should be shown. 5 . Have the applicant show the existing 46 contour. 6 . All proposed structures should be so labeled. 7 . The catch basin at junction of 24" RCP, 18" CHDPE and 48" CHDPE' s appears to be insufficient to handle the proposed 18" and 48" pipe connections on one wall . 8 . Details required for manhole, catchbasin and manhole frame and cover. 9 . Riprap should be dimensioned and the design calculations submitted. 10 . A gravel base may be required under the proposed headwall . McCahill, Zoning/Wetlands Enforcement Officer Re: Application #93-43W, Richard Grigorian DpCUMENTS REVIEWED: Application form, drainage calculations, and plan prepared for Richard Grigorian dated 9-30-93 CPMMENTS: 1F Item G, of the application form, does not indicate that abutting property owners have been notified by certified mail . The applicant should submit proof that this has been done. Section 7 . 3b, of the Regulations, states letters shall be mailed within seven ( 7 ) calendar days of the time an application is submitted. 2 The map submitted, to show the entire site with building layout (sheet 1 of 1, dated Feb. 1973 ) , does not include a recently constructed addition on the southerly side of the property. The current site layout (Town Clerk' s map #2211A) should be submitted With this application. 3': Mark W. Friend, Soil Scientist, has flagged the wetlands on this property. The mapping is unclear as many of the flags are shown just off the centerline of the watercourse . This should be clarified with regard to how wide the wetlands associated with the watercourse actually are. A report should be submitted by the Soil Scientist to verify the limits as shown and to describe the wetlands in the area. There are also wetlands associated with the watercourse at the outlet end of the existing 48" RCP. This should be shown on the plans . 41''iI A note on the plans indicates, "fill to be clean material with no bit. conc . or concrete rubble or construction debris" . This statement is in keeping with Section 2 . 1b, of the Regulations, which defines "clean fill" . There is an area near the outlet end of the existing 48" RCP where several loads of concrete have been deposited. The plans should specify what will happen with the concrete rubble. 5� The plans indicate all the trees are to be removed in the area of the swale. It would appear that some of the trees can remain. Clearing limits should be revised on the plans if possible. E izabeth A. Warren - 2 - October 21 , 1993 Application #93-43W i 6 � There are a number of junk tires in the swale. These should be removed prior to any placement of pipe or fill material . This should be noted on the plans . 7 ; The regulated area (within forty (40 ) feet of wetlands or eighty ( 80) feet of a watercourse) should be shown on the plans . 8t A detail should be provided for the proposed headwall as well as the various structures (manhole, catch basin, etc. ) . Mr. Richard Grigorian is applying for permission to conduct regulated activities in conjunction with the piping of an existing watercourse allowing the gully associated with the watercourse to be filled. The project location is shown on the maps submitted with the application by Aeschliman Land Surveying. The wetlands on the property were flagged by me on September 3, 1993. The delineation was from high intensity mapping using a dutch auger. Blue flags were used to demarcate the wetland boundary. The flag numbers for the boundaries were from 1 to 15 and B1 to B5. Flag numbers 1 to 6 and B1 to B5 demarcate the edge of the wetlands and flags 7 to 15 demarcate the centerline of the watercourse. A majority of the area I flagged was a watercourse of which the underlying soils were alluvial. The Agency shall evaluate the application pursuant to these regulations and grant the application as filed, grant it with any terms of limitations, or deny it; and shall state upon the record the reason(s) for its decision. In the event that you are requesting an extension, section 8.1.6 of the Zoning Regulations establishes the process for monitoring the approved conservation plan. Per that regulation, a progress report is required from the applicant's engineer verifying that erosion and sedimentation control measures have been installed and maintained in accordance with the approved conservation plan. This verification report shall also be submitted 65 days prior to the expiration date of the permit to allow time for the Commissioners to evaluate your project controls. If work has been finished or will be finished by the expiration date, please send a letter stating work is completed according to the approved plan.
