RE: 524 Sullivan Avenue Site Plan Review
Dear Mrs. Warren:
This is a proposed extension of a building that was presented
before the Commission in 1985, #338 .
It appears that there would not be any encroachment of the
wetlands as shown or adverse effect of this expansion.
With respect to the conservation plan; by their figures, they
expect to disturb .54 acres with the building and parking lot.
If the soil erosion and sediment control notes are followed on
this flat site, there shouldn't be a problem.
If an approval were to be granted, the normal conditions with a
$4 , 000 Bond would be appropriate to assure compliance with their
erosion control plans.

Proposed curb lines and pavement width should be
indicated on plan.
Section 8 OFF-STREET PARKING
An error has been made in the addition of required
parking spaces for the building addition. The correct
number of required parking spaces should be 14.
GENERAL COMMENTS
1 A construction exit pad should be indicated at the
entrance to the proposed parking area on the topographic
map.
2. The temporary topsoil stockpile area adjacent to the
proposed catch basin in the east corner of the parking
area should be relocated.
3. Sullivan Avenue should be labeled on the topographic
map.
4. The soil type symbol for Ninigret is incorrect on the
topographic map. The correct symbol is "NnA."
5. Soil types and classifications should be listed on the
topographic map.
6. Why are the existing catch basin outlet elevations higher
than the inlet elevations? Are these "as-built"
elevations?
7. The engineer has provided a hydrologic study for existing
and proposed conditions for the retention basin. The
retention analysis indicates an outflow increase of 0.1
cfs for the 5 and 10 year storms. Maximum water
surface elevations have increased between 0.1 and 0.3
feet for the given range of storm frequencies. Pre-
development (prior to construction of existing building)
peak discharges have not been included and, therefore
outflows cannot be properly evaluated. Although
outflows determined in this analysis are minor, it is
recommended that pre-development flows be provided.
Mr. Michael J. Gantick, P.E.
S E A Consultants Inc. November 24, 1987
Engineers/Architects Page Three
8. The engineer should indicate the type of outlet structure
and method used for determining outflows for the
rentention analysis.
9. A table listing temporary and permanent vegetative
cover including types, dates and application rates should
be provided.
10. The Town requires a minimum cover of two feet over
all drainage pipes. The two catch basins located in the
proposed parking area should be revised to meet this
requirement.
If you have any questions, please do not hesitate to call me.

RE: Engineering Review
524 Sullivan Avenue
Pursuant to my December 2, 1987 transmittal to you, please find this
Departments comments concerning the subject application:
1. Per the SEA comments, existing pipe grades suggest a poor
drainage condition from a maintenance and functional point
of view. The proposed drainage also implies the install-
ation of drainage pipe with as little as one foot of cover.
Drainage pipe material may not meet loadings. The water
level indicated in the pond would imply a backwater condition
which again will present drainage problems on the site.
2. The existing drainage (dry wells) should be (as-builts) shown.
3 . South property corner pin missing.
4 . Drainage to rear of (north) at proposed building does not
allow for sufficient room for maintenance. Also pipe slope
below minimum 0.5% grade.
5. Grading along eastern portion of proposed driveway/parking
not clear.
6. The water level as indicated in the pond and a CB to the
southeast of the property would suggest additional design
consideration for the building and parking areas including
underdrains and additional free draining fill material.

Subject : 524 Sullivan Avenue
Dear Mr . Charbonneau:
On June 28. 1988 I met with your son Kevin and Mr . Ralph Zahner
to review the drainage changes we had reviewed the previous week .
Prior to leaving the site I observed that some process gravel had
already been placed in the area of the new parking lot . I noted
to both your son and Mr . Zahner that the subgrade did not appear
to be prepared according to the approved plan, i .e . 12 inches of
gravel and 4 inches of processed stone. We all reviewed a copy
of said plan to confirm the proposed base structure. I stated to
your son and Mr . Zahner that I expected the approved base
structure to be installed . I also stated that if they felt a
change was appropriate, soil testing would be required. I also
stated that I expected a request for inspection of the subgrade
prior to gravelling and inspection of the gravel prior to paving
as per the procedures outlined in the preconstruction meeting
held on March 7 , 1988 for this project .
On July 15, 1988 I was surprised to find the parking lot was
paved. A check with Engineering Division personnel , found that
no request for inspection had been made and therefore no Town
inspection was made of the parking lot construction.
By this letter be advised that prior to the issuance of a
certificate of occupancy, proof of materials of construction of
the parking lot along with as-builts will be required .
