r
PCO No. 178 Loading Dock Structural Repairs
The Contractor shall provide all necessary labor, materials and equipment

Headworks and Control Bldg.
Reason for the Change
PCO No. 172 Railing In-fills at Anoxic Tanks
PCO No. 156 required removal of 2 existing walkway bridges for the
former Mechanical Aerators (of the old Aeration Tanks)which were not
designated for demolition. In addition, per the original contract documents
the two northern walkway bridges were designated for demolition by the
Contractor. The documents made no provisions to furnish new in-fill
railing sections at these four locations. The cost presented herein
represents the cost to furnish and install various sections of 2 line
aluminum railings at these locations at the Anoxic Tanks which were the
converted Aeration Tanks.
PCO No. 174 Low Flow Air Switch
RFI No. 278 was presented to the Engineer requesting direction on the
electrical wiring required for the low flow air switch on the Odor Control
fans. Contract documents did not make provisions for this wiring. The
cost presented herein represents the time and materials required to furnish
conduit and wiring for the Odor Control system low flow air switch.
PCO No. 175 SITYP High Pressure Switches
RFI No. 275 was presented to the Engineer requesting direction on the
SITYP pump manufacturers discharge high pressure switch for each pump.
Contract documents did not make provisions for this conduit and wiring for
the switch which was provided by the manufacturer. The cost presented
herein, represents the time and material required to furnish conduit and
wiring to provide a high pressure discharge alarm for each SITYP pump.
PCO No. 176 Glass Replacement at Control Building
The former laboratory was located on the first floor of the Control Building
in the northwest corner. The upper west window elevation had contained
. f
i
panes which did not have glass in them due to renovation work previous to
this contract. Architectural drawings did not call for installation of new
glass sections where pieces of plywood were currently installed. The
Contractor was requested to provide a cost estimate to install four new
glass sections at the west window elevation of the Control Building.
PCO No. 177 Replace two Catch Basin Tops
During reconstruction of an existing storm catch basin and repair of an
existing 12" storm pipe at the west entrance gate, the Contractor submitted
RFI No. 208 inquiring on the disposition of two existing catch basin tops
west of the Administration Bldg and in the center of the entrance. The
Owner directed the Contractor to replace the two existing catch basin tops.
This was performed on a time and material basis and the costs are
presented herein.
PCO No. 178 Structural Repairs to Loading Docks
Contract documents indicated to make minor structural repairs to the two
concrete Loading Docks at the Headworks and Control Bldg. The Owner
requested complete removal and replacement of these concrete docks.
CDM Smith Structural Engineers visited the site in October and prepared a
report on the condition of both docks. Their report did not warrant full
replacement of the structures, however due to the time elapse between
original design and construction, additional repair work and crack injection
was required. The Contractor was directed to perform the additional
repairs on a time and material basis. The overall total cost includes a credit
for the original repair work which was included in the Contractor lump
sum bid.

additional cost of$4,352.84 for the installation of aluminum railing in-fills at the Anoxic Tanks where the

The revised amount, $3,857.00(rounded up)will be included in a forthcoming Change Order for the

Response to RFI No.275
The pressure switches mentioned in RFI No. 275 are currently mounted in the field.
Install 2 No. 14 wires from each switch back to PLC-CB. Discussions with WJGE,they can utilize spare conduit for
some of the distance between the chemical room and the electrical room and will provide a new conduit drop to
each switch. The contact from each switch shall provide a "High Pressure SITYP Pump No.1"and "High Pressure
SITYP Pump No.2"alarm at PLC-CB, utilize 2 spare digital inputs at PLC-CB. CDM AE group will write software to
shutdown the pump upon initiation of the switch contact.A COR will follow.

Subject. South Windsor Water Pollution Control Facility:Loading Docks
Introduction
On October 11,2011,the condition of three loading docks at the South Windsor Water Pollution
Control Facility was assessed. One loading dock is located along the east side of the Headworks
Building-and two loading docks are located at the Control Building, one on the east side and the
other on the west side.
The reinforced concrete loading dock at the Headworks Building was constructed around 1980
and is approximately 24 feet by 4 feet in plan. The concrete slab is of cantilevered construction
and has a thickness of 8 inches at the face of the wall tapering to 4 inches at the edge.
The two reinforced concrete loading docks at the Control Building were constructed around 1966
and are of cantilevered construction. The east loading dock is approximately 46 feet by 4 feet in
plan, with a thickness of 8 inches at the face of the wall tapering to 4 inches at the edge. The west
loading dock has plan dimensions of 16 feet by 3 foot-8 inches and a tapered slab thickness of 8
inches at the face of the wall to 4 inches at the edge.
The loading docks were previously assessed in December 2007. Photographs from that
assessment are used in this report as comparison to the present day condition. Visual
observations and hammer sounding of the top and underside of the concrete slabs were the
means used to determine the condition of the existing loading docks. Several photographs were
taken for documentation.
Observations
Specific areas of deterioration were noted as follows:
Headworks Building-East Loading Dock
Steve Kasle
10/25/11
Page 2
Top Side of Slab:
■ There was a new 1-ft by 3-ft area of spalled concrete approximately 3-in deep along the
edge of the south end of the dock. Within the spall, there were two exposed reinforcing
bars with surface rust, approximately 3-ft long. There was a new 2-ft by 1-ft area of
hollow sounding concrete adjacent to this spall.
■ Adjacent to the column at line 3,there was a new area of spalled concrete approximately
3-ft by 3-ft by 1 Ya-inches deep. There was an existing spall, 12-in by 8-in by 1-in deep,
directly across from the column at line 3 as previously indicated in 2007.
■ Adjacent to the column at line 6,there was an area approximately 3-ft 8-in by 4-ft 6-in by
1 Y2-inches deep (maximum) of spalled, hollow sounding concrete. There was an area
within this spall that had been previously repaired;however, the repair was not bonded
to the underlying concrete.
■ The north-east corner of the dock had a new spalled area of loose concrete approximately
6-in by 10-in by 2-in deep.
■ There were two new cracks toward the north end of the dock approximately 3-ft apart
running in the short direction. One crack was 3-ft long by%-in wide and the other crack
was approximately 4-ft long by 1/8-in wide. Both cracks were continuous through the
concrete slab to the underside.
Under Side of Slab:
■ There were three new areas of spalled concrete, totaling approximately 7 square feet by
1-in deep, located along the east edge of the loading dock. Within these spalls, there were
approximately 9 linear feet of exposed reinforcing bars with surface rust.
t There were 5 new cracks (approximately 1/8-in wide)totaling approximately 10 linear
feet running in the short direction of the slab span. There was a rust stain on the
underside of the slab along the crack and efflorescence was visible in all cracks.
Control Building-East Loading Dock
Top Side of Slab:
■ There were three areas of spalled concrete totaling 4 square feet by 1-in deep located on
the top slab. One of the areas, 10 inches by 8 inches in size, was a new spall which
developed after the 2007 assessment.
Under Side of Slab:
■ There were three new areas of spalled concrete, totaling approximately 5 square feet by
1-in deep, located along the east edge of the loading dock. Within these spalls, there were
approximately 9 linear feet of exposed reinforcing bars with surface rust.
■ There was 1 new crack(approximately 1/8-in wide)totaling approximately 2 linear feet
running in the long direction at the east side of the dock. There was a rust stain on the
underside of the slab along the crack and efflorescence was visible in all cracks.
Steve Kasle
10/25/11
Page 3
■ The bottom flange of the channel was rusted.
Control Building-West Loading Dock
Top Side of Slab:
■ No deterioration on the top of the slab.
Under Side of Slab:
■ There was one new area of spalled concrete approximately 6-in by 6-in by 1-in deep
located 4-ft from the south end of the dock. There were 2 reinforcing bars with surface
rust totaling 1 linear foot within the spall.
Discussion and Recommendations
Headworks
Some attempt has been made to repair the loading dock since the inspection in December 2007.
These have broken up and the loading dock shows signs of further deterioration. Photographs 1
and 2 were taken of the top side of the loading dock during the first inspection in 2007. The
previous repairs and additional spalled concrete areas are visible.in Photographs 3, 4, and S
taken in October 2011 at the same location.
CDM recommends repairing the top and underside spalls and cracks as follows:
1. Remove the loose, previous repairs down to intact concrete.
2. Remove the spalled concrete as specified.
3. Where reinforcing steel is exposed within the spalled concrete, the reinforcing steel
should be cleaned and incorporated into the new repair mortar as specified.
4. Repair the cracks by injecting epoxy adhesive.
S. Roughen and clean the substrate concrete as specified.
6. Install new polymer modified repair mortar to restore the concrete members to their
original configuration as specified.
Application of the anti-skid coating indicated on the architectural drawings will prevent further
ingress of moisture and salt, and should stabilize the condition of the loading dock.
Control Building
Observations of the top side of the East loading dock indicated only one small additional area of
deteriorated concrete since the 2007 assessment. Deterioration was not noted in the 2007
assessment on the underside of the dock, however, the recent site inspection indicated a few

areas of spalled concrete. The concrete cracks and spalls noted on the underside of the slab
should be repair as stated above.
The West loading dock at the Control Building indicated one area of spalled concrete on the
underside of the slab. This spall should be repaired as stated above.
